NOTICE OF DISTRICT TRIAL – TRIAL ROUND 1

·  This form outlines the relevant information for all of the sports conducting trials in Nambour District Trial Round 1.

·  All students must collect a Nambour District “District Trial Permission Form” from their Sports Co-ordinator / PE Teacher.

·  This Permission Form must be completed in full by parents/guardians and signed by your School Principal/School Delegate.

·  Students must bring this signed Permission Form to the Selection Trial listed below. NO PERMISSION FORM = NO TRIAL. There will be NO EXCEPTIONS.
