Steps to Follow:

/ 0) Get on the web and find this form.1) Download and print this form., or type answers directly on it for emailing.
2) Answer the questions on this form.
3) Fill in the schools with the due dates in the box on the first page.
Then hand in this form and get my OK that you’re on the list.
By October 1 (ED/EA/Rolling/Priority)
Or Nov 1(Regular) / 4) For each school prepare:
- The recommendation form with all the top info filled out, including my name & address.
- A stamped envelope properly addressed to the college.
- A draft of your major essay for that school.*
* I am willing to entertain essays after these deadlines so long as you submit at least ONE before the deadline.
Rest of the year
/ 6) Do your job by continuing to get good grades and by writing awesome, tremendous, and astounding college essays, and WARN ME if somehow you’ve blown it.7) Please inform me of any changes
(e.g. additional schools, finally decided which school you are applying to early…)
8) ESPECIALLY inform me of any disasters (illnesses that affect your grades, abrupt changes in family/financial aid situation, etc.)
9) Fill out your FAFSA.

Questions, questions, I have lots of questions! Please answer honestly and in detail (avoiding vague generalities or empty superlatives, unless you’re hoping I’ll use those in my letter).
1)What areas are you interested in pursuing in college? How do you think math (for math students) or what you learned in PALS (for PALS) (and both for both, smarties!) will help you in those pursuits?
2)Give two personal characteristics that you value. Explain how these characteristics have helped you in life, in your studying, and/or in my class.
3) What non-academic activities have influenced how you approach school, life and learning? What do you enjoy doing when you are not experiencing the thrill of studying mathematics? What have you learned from them or how do they help you?
4) What significant and specific events happened in my class (or in PALS) that had a positive affect on you? What did you learn most about yourself by taking this particular course with me? Don’t just wax poetic about generalities.
5) Is there something else about you that you’re hoping will be reflected in my recommendation—something you’re hoping I’ll bring up or address? Be specific about what it is and what you want me to say about it. (I don’t promise that I will say it, but I’ll certainly consider it!)
