Complete the form for each person attending the conference,save the file (Please “save as” and add your own last name to the file name, for example “Registration Booking Form Brown.doc") email the file to:

CANCELLATIONS
Request to cancel must arrive in writing by 15 May 2012, and you will be reimbursed for 50% of the registration fees, and the accommodation package reduced by 60 euro.
After this date no reimbursement can be made.
