MILTON ADULT HOCKEY LEAGUE
Waiver and Indemnity
I AGREE:
1. That at all times during the 2008-2009 Season of the Milton Adult Hockey League my safety remains my sole responsibility and that I will observe all rules of the League; and
2. That I will wear protective equipment and an approved helmet while participating in the League and that my participation in the event may be terminated at any time by the League Officials for failure to do so; and
3. That I am aware of the risks inherent in participating in the League and assume all such risks.
IN CONSIDERATION of the acceptance of my application and permission to participate as an entrant in the League, I for myself, my heirs, administrators, successors and assigns HEREBY RELEASE, WAIVE AND FOREVER DISCHARGE, Milton Adult Hockey League, League Officials, the Town of Milton and all other associations and sponsoring companies and all their respective agents, officials, servants, conductors, representatives, successors and assigns OF AND FROM ALL claims, demands, damages, costs, expenses, in respect of death, injury, loss or damage to my person or property HOWSOEVER CAUSED, arising or to arise by reason of my participation in the said League AND NOTWITHSTANDING that same may have been contributed by negligence of any of the aforesaid. I FURTHER UNDERTAKE TO HOLD HARMLESS AND SAVE HARMLESS and AGREE TO INDEMNIFY all aforesaid from and against any and all liability incurred by any or all of them arising as a result or in any way connected to my participation in the said League. Personal information, including full name, will be publicly displayed unless otherwise requested.
BY SUBMITTING THIS ENTRY I ACKNOWLEDGE HAVING READ, UNDERSTOOD AND AGREED to the above AGREEMENT, RELEASE, WAIVER AND INDEMNITY. I WARRANT that I am physically able to participate in this event.
The Milton Adult Hockey League and the Town of Milton has a strict no alcohol policy within Town facilities and adjacent property. Any person or teams found with alcohol will be removed from the league with no refund of fees.
