Device Loan Agreement

2016-2017

Students and parents/guardians participating in the Middleborough Public Schools in the laptop/tablet initiative must adhere to the program procedures, Student Code of Conduct, Student Handbook, Technology/Network Acceptable Use Policy, School Committee Policies, and federal and state laws.

The technology device covered by this agreement is an Acer Aspire Switch 10 tablet. One Acer Aspire Switch 10, power cord and case are being loaned to the student. It is the student’s responsibility to care for the equipment on a daily basis and ensure that it is stored in a safe place. Any technical or mechanical issues with the Acer Aspire Switch 10 tablet must be reported to the school for evaluation and repair.

This equipment is, and at all-times, remains the property of the Middleborough Public Schools, and is subject to random inspection. Students should be aware that all activities on a district device is monitored. The equipment will be returned to the district during the final week of school or sooner if a student withdraws from the district.

Student Responsibilities

Your Acer Aspire Switch 10 tablet is an important learning tool and is for educational purposes. In order to take your Acer Aspire Switch 10 tablet home each day, you must be willing to accept the following responsibilities:

·  I understand that the Acer Aspire Switch 10 tablet is my responsibility.

·  I will not leave my Acer Aspire Switch 10 tablet in unsupervised areas, and will do everything I can to prevent it from being damaged.

·  I will honor the Middleborough Public Schools Acer Aspire Switch 10 tablet Procedures and Guidelines and the Technology/Internet Acceptable Use Policy.

·  I will not view, create, send, or download inappropriate material.

·  I will bring the Acer Aspire Switch 10 tablet to school every day with a fully charged battery.

·  I will treat the Acer Aspire Switch 10 tablet as a valuable piece of equipment and will report any mechanical or technical issues through School Dude.

·  I will not attempt to add, delete, access, or modify other user accounts on the Acer Aspire Switch 10 tablet.

·  I will take no action that could interfere with the Middleborough Public Schools’ network.

·  I will return the Acer Aspire Switch 10 tablet in good working order at the end of the school year, when requested, or upon withdrawing from the school district.


I agree to the terms above and accept responsibility for the Middleborough Public Schools-owned Acer Aspire Switch 10 tablet. I understand that I am accepting the responsibility for any malicious damage, destruction, or loss of the assigned Acer Aspire Switch 10 tablet. I understand that I am responsible for the total cost of replacing a damaged, destroyed, or lost Acer Aspire Switch 10 tablet due to malicious or negligent actions.

Parent/Guardian Responsibilities

Your child has been issued an Acer Aspire Switch 10 tablet as part of the Middleborough Public Schools initiative to enhance the educational experience as outlined in the Acer Aspire Switch 10 tablet Procedures and Use Guidelines document. It is essential that all procedures and guidelines be followed to ensure the safe, efficient, and ethical operation of this device. Therefore, we ask that you agree to the following:

·  I will supervise my child’s use of the Acer Aspire Switch 10 tablet at home.

·  I will discuss the Middleborough Public Schools Acer Aspire Switch 10 tablet Procedures and Use Guidelines and the Middleborough Public Schools Technology/Internet Acceptable Use Policy information with my child.

·  I will ensure that my child understands the Student Responsibilities in this loan agreement.

·  I will relay the expectations of the Middleborough Public Schools regarding the use of the Internet and email at home and will supervise my child’s use of the Internet and email.

·  I will prohibit the purchase of apps or other goods via the Acer Aspire Switch 10 tablet.

·  I will ensure that my child reports any mechanical or technical issues with the Acer Aspire Switch 10 tablet through School Dude.

·  I agree that the Acer Aspire Switch 10 tablet will be returned to Middleborough Public Schools in good working order at the end of the school year, when requested, or upon withdrawing from the school district.

☐ Opt out (Using your personal mobile device)

Students that opt out may bring only one mobile device to school to connect to the MPS network for use in classrooms. It is recommended that laptops and tablets should have a 10” screen or larger with browsing capabilities to be used for educational purposes only. Student devices will connect to the wireless infrastructure and receive safe, managed access to resources. Similar to a district owned computer, student Internet access will be filtered in accordance with the Children’s Internet Protection Act (CIPA).

*Note: If asset tag is removed please notify the Technology Department. Student cell phones are not allowed on the MPS wireless network.
