ALL CHANGES TO PRE-REG MUST BE MADE BY OCTOBER 13TH AND PAIRS SECOND HANDLER MUST BE SUBMITTED WITH PRE-REG!!!
All entries are required to be received by midnight Sunday, October 11th, 2015 in order to compete.
You can pay your entry fees the day of the event, however it is preferred that they are mailed in advance.
1. Email pre-registration forms to Melanie Griggs @ by Sunday, October 11, 2015 to avoid missing this event.
2. If mailing payment, please send by Saturday, October 10, 2015 to Melanie Griggs, 25230 Trunk Line Rd, Henderson, MD 21640.
3. Thank you for adhering to these Rules; it makes a world of difference in organizing Registration Entries ahead of time.

3. Spot Landing
4. Bullseye
5. Time Trial

Notes:
a) Please identify your Pairs D/A partner on the Pre-Reg form or request to be paired up
b) No Skyhoundz Hyperflite Divisions
c) World Qualifier – Top Non-Qualified Team (Handler(s)/Canine) for each Event/Game (ie 5 World Qualifiers at each competition … 1 Pairs Distance/Accuracy, 1 Freestyle, 1 Spot Landing, 1 Bullseye & 1 Time Trial)
d) Max of 2 Qualifying Spots allowed per person at any given World Qualifying Event
Note: 1st Place determines the qualifying teams unless the team has already qualified. In this scenario, the qualification will roll down to the next placement team that has not already qualified or meets the Max Qualifying Spots Rule above. Also, the entire team is the basis for who has qualified, so if any part (Handler(s) or Canine) of the team differs they will be considered a different team and are eligible to qualify.

MicroDog Division – under 17 " at the withers.
Note: The Team (Handler/Canine) can only enter in one MAD Dogs’ Division for all Toss & Fetch Games. Teams are able to enter a different Division for Freestyle if they so choose (AA-Advance, A-Intermediate, B-Beginner, MicroDog or Youth-Under 15 Yrs). Also, as a club member, your Division remains constant for the season in order to determine Club Cup Points.

Release and Waiver of Liability
For and in consideration of my participation in the, Skyhoundz Hyperflite, and Mid Atlantic Disc Dogs event, and for other good and valuable consideration, the receipt and sufficiency of which are hereby acknowledged, I the undersigned, intending to be legally bound do hereby for myself, my heirs, assigns, executors and administrators, waive and release any and all rights and claims for damages or injuries arising out of, or in any way related to, my participation in the Event, which I may have against the following parties: Skyhoundz Hyperflite, and MAD Dogs, the administration and owner of the facility hosting the Event; together with the contractor, agents, employees and representatives of each of them. I certify that my canine and I are healthy and fit to compete in the Event.
I hereby grant to, Skyhoundz Hyperflite, and MAD Dogs the exclusive right to record, broadcast, photograph, film and videotape my performances and related interviews at the Event and to utilize such recordings, broadcasts, photographs, films or videotapes together with my name, image, likeness, voice, and biographical information, for such publicity, promotional or commercial purposes as they may desire. I understand that, Skyhoundz Hyperflite, and MAD Dogs may assign the right to use the recordings, broadcasts, images, photographs, film and videotape of my performance(s) together with my name, image, likeness, voice and biographical information to such person or other entity (including, but not limited to a television broadcaster) as they may desire without my further consent or knowledge and I expressly agree to such assignment.
