Michigan Annual Health Coverage Renewal

If you are enrolled in state health care coverage (Healthy Michigan Plan, Aged/Blind/Disabled Medicaid, MI CHILD, Healthy Kids etc…), you are required to complete health care coverage renewal paperwork at least once every 12 months. This worksheet will give you information on what steps you must take atyour annual renewal (redetermination).

Your renewal letter (also called a Redetermination notice) should arrive in your 11th month of coverage.

Typically,the month of your renewal will be based on the date your Health Care coverage began.

  • Example: You applied for coverage on April 13, 2014 and were approved for coverage on May 5, 2014. Your coverage start date wasApril 1, 2014. DHS will send your renewalletterin February 2015. Your renewal paperwork will be due on March 1, 2015.

To complete your renewal online, visit You will log-in using the MI Bridges username and password you created when you applied. If you do not have an account, you can create one.

Your renewal due date is listed on the first page of your renewal letter and forms. If you need more time, contact your DHHS Specialist and request an extension.Your Specialist can grant up to two 10 day extensions;howeveryou may not be notified that your request was granted.

Helpful Tips:

  1. If you can’t reach your specialist via phone to request an extension, send an email so that you have proof of the request. Your specialist’s email address is the Specialist ID (located on the top right corner of any letter you get from DHHS) followed by @michigan.gov.
  1. DHHS may request income verification for the last 30 days for all types of income.
  1. DHHS may request proof of your assets. This may include: bank accounts, land, cars etc…
  1. If DHHS requests bank account proofs you must send copies of bank account statements. You will needask your banking institution to print a copy of your account information showing your last month of activity, account balance, account number, and account owner’s name.
  1. Always include your case number or full name on your proofs.Make copies of your proofs, do not send originals to DHHS.

If you miss the deadline to return your renewal paperwork, you will receive a notice informing you of the date your health care coverage will end.

If you lose your coverage, you can reapply at any time through your MI Bridges online account, in person with a local assister, through your local DHHS office, or through Healthcare.gov.
