Metromont is committed to helping you improve your health and supports a culture of wellness. Our Wellness Program is available to both you and your spouse (if enrolled in Metromont’s medical plan) through TargetCare’s “Right-On-Target” Program. By participating in the Right-On-Target Program, you have the ability to receive an employer contribution to your Health Savings Account (HSA).
Through the Right-On-Target Program, you and your spouse receive a free annual Clinical Health Assessment (CHA) that is designed to help raise awareness of certain health risk conditions. You also have a detailed review of your CHA results with a TargetCare health coach and a structured visit schedule to help you stay on track towards reaching and maintaining your health goals. If you work at a location that has an Onsite Health Clinic, you can visit the nurse in person. Associates working in Bartow and Spartanburg (and their participating spouses) will hold their visits telephonically.
The results of your CHA will be identified on your TargetCare Risk Summary report. On this report, you and your spouse will be placed into one of the following risk categories and be required to visit with the TargetCare health coach the number of times per year as per your designated group:
Risk Category / Clinic Visit ScheduleExtremely High Right (>100) / At least every 4 weeks
High Risk (81 – 100) / At least every 8 weeks (or more)
Borderline High Risk (61 – 80) / Every 8 weeks
Above Normal Risk (41 – 60) / Required to meet with the TargetCare health coach within 16 weeks from your initial CHA and biometric screening
Optimal/Normal Range (0 – 40) / No additional visits required outside of your initial CHA appointment and biometric screening; however, you are still encouraged to speak with the nurse at any time to discuss maintaining your optimal status or about any new health concerns
By you and your spouse meeting the requirements of the program, you can receive a contribution from Metromont of $100 per quarter (up to $400 per year) to your HSA. For 2017, your eligibility to receive the employer HSA contribution will be based on the following:

If you are hired after January 1st, your spouse is not required to have a CHA until the next available CHA event. Only you are required to be compliant with the Right-On-Target Program by having a CHA, a health coach review, and visit with the TargetCare health coach as required by your risk category.
In order to be eligible to receive the employer HSA contribution, you must be enrolled in Metromont’s medical plan for at least a full quarter and have a Health Savings Account through HSA Bank in your name. You must also be employed at the time the HSA contribution is awarded, which is usually the 1st pay after the end of the quarter.
It is your responsibility to visit with the TargetCare health coach as required. Should you or your spouse not meet with the health coach as scheduled, you will be designated as non-compliant with the program and not eligible to receive Metromont’s contribution to your HSA.
TargetCare will determine whether or not you are compliant with the Right-On-Target Program. Appeals regarding your compliance status can be made to TargetCare only. Metromont will not make any determination regarding the compliance status of its associates.
