Attention Massapequa Service Provider:

In order to establish an increased coordination of services for our students in Massapequa School District, we want to outline specific parameters as to when services can be provided. Accordingly, the following guidelines should be adhered to in order to fulfill the services mandated on each student’s IEP:

□At this point, each therapist should have established a set schedule with each student they are servicing.This schedule must reflect the exact frequency and duration of the services stated on the IEP. (ei. If the IEP states 4x 60 weekly, you must do 4 one hour sessions on 4 different days; if the IEP states 3x90, you must do 3 – 90 minute sessions per week on 3 separate days. You must adhere to this schedule. You may not change the frequency or duration of the sessions, they must match the IEP exactly. If you need to reschedule a session during the week, you must document a Teacher Absence on the scheduled date and a make-up for the actual date of the session.

□All IEP mandated services are provided according to the School District calendar (please visit the Massapequa Website at to view the calendar). Therefore, if school is not in session on one of the days you are scheduled to provide services, you are not to provide services on that day. On your log sheet you are to use the “H” code, indicating that the session fell on a holiday. There are no “make-ups” for sessions that fall on a day in which school is not in session nor can the session be rescheduled during the same week.

Massapequa will not accept any additional billing. So you must submit your Massapequa billing on time, we cannot accept any additional billing for Massapequa. Additional billing is billing received after the billing cycle for the month is completed, which is the 15th of the following month. (ex. September billing for Massapequa will not accepted after October 15th). Remember billing is due on the 3rd and the 18th of the month, for checks released on the 19th and the 31st.

□If the Massapequa School District closes for a Snow Day (or other weather condition or emergency), services cannot be provided. If the School District cancels their after school activities, home services after school hours cannot be provided. They need to be marked with an H for holiday. These sessions cannot be made up or rescheduled.

Any sessions that arecancelled by the parents cannot be made up or rescheduled. If you show up for a session and no one is home (or school – location of service), you should record that in the session notes, but it is NOT a payable item, it is a CA. If you (the provider) have to cancel a session, it can be made up within two weeks of the cancelled session. All make-up sessions are to take place on days in which school is in session as per the Massapequa School District calendar. Make-up sessions are not to take place on Saturdays and Sundays. If a therapist provides services on Saturday or Sunday and logs it as a weekday session, this is considered falsification of the student’s record and subject to disciplinary action. Additionally, if a child is absent from school, home services cannot be provided. Sessions that are missed for this reason cannot be rescheduled or made up.

□Parent Training monthly sessions have more flexibility. There are no make-ups or rescheduled Parent cancellations.

□When there is an absence, it must be recorded on your log notes accordingly (CA – child absence and TA – therapist absence). Make-ups for Teacher Absences can be done within 2 weeks of the missed session. When there is a make-up for a TA, you must indicate the date of the TA you are making up.

  1. Your logs should read this way for a child absence: CA – session note:“child absent”
  2. Your logs should read this way for a therapist absence: TA – session note: “therapist absent; MU scheduled or unable to schedule MU at this time”
  3. Your logs should read this way for a make-up: MU – session note:“MU for TA on x-date”
