Request for Special Meals

This request must be fully completed and approved prior to each event. The meal cost per person should not exceed the PPM 49 meal allowances per participant. Please refer to the LA state travel guide for meal allowance rates.

This form is submitted (scanned/emailed) to the Northshore Technical Community College (NTCC) VC Finance & Administration after the Dean of Campus Administration has signed and reviewed that the form was fully completed. This form should be submitted to the VC Finance & Administration at least one week prior to the planned event. This will allow enough time to obtain the proper approvals necessary. A copy of the attached sign-in sheet with printed names, signatures, title, and affiliation should be scanned/emailed to the NTCC purchasing office for each day of the event. Form FIN 017 should be fully completed and submitted in both electronically scanned AND MS Excel formats within two weeks of the date that the special meal was provided by the corresponding campus. A purchase requisition must be sent to the purchasing office prior to each event to be in compliance with state purchasing regulations. If the total cost of the meal is in excess of $5,000, three bids must be received by the NTCC purchasing staff and the lowest bidder used. The original invoice from the vendor, the requisition, and the sign-in sheet(s) must be sent to the purchasing office in order for payment to be made.
