About Oxford Deanery

The Oxford Deanery covers the counties of Oxfordshire, Berkshire and Buckinghamshire, with Banburyand Milton Keynes in the North, to Readingand Slough in Berkshire and High Wycombe and Aylesbury in the West.

The Oxford Deanery is part of the South Central Strategic Health Authority which serves a large population and covers the regions mentioned above as well as the Wessex Deanery areas of Hampshire and the Isle of Wight. The Oxford Deanery is responsible for the training of some 1500 trainees.

The General Surgery Training Placement

The General Surgery training placement is for a period of four months starting at ST3 Level. During this time, the trainee's work will be monitored for satisfactory progress and subject to annual reviews in the form of ARCPs.

The posts on this rotation have been approved for Specialist Training by the Royal College of Surgeons. The Postgraduate Dean has confirmed that this placement has the necessary educational and staffing approvals.

This placement is based at Buckinghamshire Healthcare NHS Trust. The base hospital is Wycombe Hospital and Stoke Mandeville Hospital, Aylesbury with some outpatient activity at Amersham Hospital.)

Trust Information

Buckinghamshire Healthcare NHS Trust provides a wide range of high quality acute and community services from three acute hospital sites in Amersham, Stoke Mandeville and Wycombe, five community hospitals at Thame, Marlow, Buckingham, Chalfonts and Gerrards Cross and Waterside, and a number of community sites across the county. Specialist services include our regional dermatology, allergy and skin cancer centre, burns care and plastics sub-regional centre, cardiac services and the National Spinal Injuries Centre.
More than 6,000 staff serve residents across Buckinghamshire, Thame (Oxfordshire), Tring (Hertfordshire) and Leighton Buzzard (Bedfordshire) - a combined population of 500,000. It serves a much larger population, 1.5m for burns and plastic services and 14m for spinal injuries.

Stoke Mandeville

Stoke Mandeville Hospital is an acute hospital located on the edge of the market town of Aylesbury. It provides a range of hospital services including 24 hour accident and emergency, maternity, cancer care and a range of outpatient services. The hospital is the base foreye carefor the area.The regional burns and plastics unit provides specialist services to patients from Buckinghamshire, and as far away as Reading and Northampton. Stoke Mandeville Hospital is also home to the world renowned national spinal injuries centre.

Wycombe

Wycombe Hospital is situated in the centre of the historic town of High Wycombe, and offers a full range of planned surgical services, as well as 24 hour emergency medical care (including minor injuries) and specialist medical care, including stroke and heart conditions. There is also a midwifery-led maternity unit. The hospital also offers specialist cancer and urological services.

Duties of the Post

This post would be for four months and rotates internally with four other Surgical Registrars normally on a six monthly basis whilst attached to two surgical firms.

The duties if the post include ward rounds, theatre sessions, teaching, administration, research and care of inpatients and outpatients primarily at WycombeHospital and StokeMandevilleHospital, but also at AmershamHospital.

All emergency surgical work is centred at StokeMandevilleHospital. The registrars run a 1:8 24hr resident on-call rota at this site. This acute site is covered at all times by a dedicated consultant (for 1 week at a time), a registrar, an SHO (2 in the daytime) and 2 PRHOs (up to 8 on a weekday daytime).

All inpatient elective operating is at the Wycombe site, although there is some day surgery at Stoke Mandeville.

The service is broken down into 3 firms of 4 consultants: Breast, Colorectal and Upper GI / Vascular. Each firm has 2 registrars, 2 staff grades/associate specialists, 2 SHOs and 1 PRHO. The majority of the PRHOs are based at the acute site.

The consultants run a 1:12 on-call where they cease all elective activity and dedicate themselves to emergency surgery for one week. Where possible the consultant works with a registrar from his/her firm for that week.

The appointee should have the FRCS and should be competent with minor and intermediate surgery.

Introduction

Buckinghamshire Hospitals NHS Trust was formed in April 2003 from the formal merger of South Buckinghamshire NHS Trust and Stoke Mandeville NHS Trust.

After the merger the Trust developed a range of clinical networks to serve the new population. There was also a period of extensive public consultation on how the services for area should be structured. This resulted in the drawing up of comprehensive plans for reconfiguration, which are currently being implemented. Included within these plans was the development, through the ‘Shaping Health Services’ project, of a Surgical Centre at Wycombe GeneralHospital. Launched on the 30th August 2005, this is a dedicated surgical facility to provide planned day case and inpatient care for a range of routine treatments. To complement this facility, all surgical trauma and emergencies were centralised at StokeMandevilleHospital. This project served to provide surgical services in a manner that reduced delays for patients waiting for theatre, maximised available theatre time, and delivered elective and emergency care in a more systematic way.

Population served

The Trust serves residents in Buckinghamshire, Thame (Oxfordshire), Tring (Hertfordshire) and Leighton Buzzard (Bedfordshire), a combined population of almost 500,000. It serves a much larger population for Burns and Plastics services (1.5m) and Spinal Injuries (14m).

Three Primary Care Trusts are responsible for commissioning healthcare for this population (except for Bedfordshire and Hertfordshire patients): Vale of Aylesbury PCT; Chiltern and South Bucks PCT; and Wycombe PCT.

Trust resources and services

There are three hospitals in the Trust – AmershamHospital, StokeMandevilleHospital, and WycombeHospital. It has 1044 beds, 4,700 staff and an annual budget of £185m.

Between them the hospitals provide a comprehensive range of acute hospital services. Stoke MandevilleHospital and WycombeHospital provide A & E services, with all emergency surgery being admitted through Stoke Mandeville. Both sites have critical care support, general medicine, and the provision of day case general surgery. All inpatient care is performed in the Surgical Centre at Wycombe, which also serves as the base for Urology and Vascular Surgery, and for the cardiac catheterisation service. Stoke Mandeville is the base for Ophthalmology, Burns and Plastics and Spinal Injury services.

Wycombe and AmershamHospitals were significantly upgraded as a result of a recent £40m PFI scheme. Stoke Mandeville’s PFI scheme includes medical wards, elderly care wards, paediatrics, day care and space for the Burns’ unit. The current value is £37m. Building work was started in 2003 and will be completed by the end of 2005.

The Trust hosts the Buckinghamshire Shared Services - an organisation providing finance, HR, IT and procurement services for the majority of NHS organisations in Buckinghamshire.

The Trust’s headquarters are based at AmershamHospital.

Educational Opportunities

The hospitals have excellent Postgraduate Centres including a first class library, CD ROM, Medicine Search facilities, internet access and a link to the British Library. There are fully equipped lecture theatres and several small seminar rooms. The hospital has a regular Audit half day each month when all the junior doctors in the hospital are expected to attend. Joint meetings are held with Pathology, Radiotherapy and Oncology and Plastics.

The appointee will meet with Mr.Huang , the Surgical Tutor on commencement of the post when study/research time will be discussed and allotted. The four colorectal consultants will share the Trust Registrar each- so you will work for four Consultants at any stage. There are regular meetings and journal clubs and individual research/audit projects are strongly encouraged. There is an emphasis on teamwork and good secretarial support. Study and annual leave should be booked at the beginning of the post and all leave forms must be signed by both a supervising consultant and the service manager. A calendar is situated in the service manager’s office and all leave should be entered on the calendar and leave forms then forwarded to Medical Staffing.

Principal Responsibilities:

The duties and responsibilities of the Trust Registrar will be appropriate for a first/second year registrar and will include theatre sessions, outpatient clinics, ward rounds, supervision of the House Officers and Senior House Officers. Although the appointee will be an integral member of the colorectal unit, they will be required, together with the other middle grades, to provide emergency surgery general surgical cover at the Stoke site. This will be on a regular rostered basis when they will work under the supervision of the on call surgical consultant.

Facilities at WycombeHospital

1.Wards

The Directorate of General Surgery and Urology have two adult wards for elective care at WycombeHospital.

Ward 12A – General Surgery 27 beds

Urology Ward – Urology 22 Beds

There are facilities in the Paediatric wards for the treatment of General Surgery and Urology elective paediatric cases.

2.Outpatients

There are two fully equipped outpatient suites one on the main site at Wycombe and another at AmershamHospital. There are additional outpatient facilities at Marlow hospital, CheshamHospital and Chalfont and Gerrards Cross hospital.

The breast firm is supported by a dedicated team of breast care nurses.

3.Theatres

There are 3 fully equipped theatres in the main complex. The day surgery unit and theatre is in the adjacent newly opened LoakesPark development, which also contains two orthopaedic and one urology theatre. There is a dedicated theatre manager who is responsible for the staffing of the theatres with specialty-trained staff. There are three further gynaecology and obstetric theatres.

4.Day surgery

There is a dedicated day surgery ward with 19 beds for all day case surgical work.

Support Services

X-Ray.

The department has access to plain radiography, ultrasound, spiral CT, MRI scanning and image intensification. We have digital imaging with high definition workstations on both the Wycombe and Amersham sites.

Pathology Laboratory

There are full facilities on site for Haematology, Histopathology, Microbiology and Biochemistry.

Intensive Therapy Unit

Each hospital has a fully equipped ITU, with 16 beds in total.

Endoscopy

There is a dedicated endoscopy suite.

Stoke MandevilleHospital

Stoke MandevilleHospital is an acute hospital with an excellent reputation built on pioneering techniques in reconstructive surgery and rehabilitation of spinal injury patients. The hospital provides acute general hospital services to Buckinghamshire, adjacent parts of Bedfordshire, Hertfordshire and Oxfordshire.

Clinical services include a 24 hour Accident & Emergency department, General Medicine, General Surgery, Trauma & Orthopaedics, ENT, Gynaecology, Gastroenterology, Paediatrics, Obstetrics, Elderly Care, Cancer Care, and Chemotherapy. The hospital has a Regional Burns Unit and well established sub-regional specialties in Plastic & Reconstructive Surgery, Ophthalmology, Rheumatology, Maxillo Facial Surgery, Orthodontics and Dermatology. Spinal Injuries catchment includes much of Southern England, East Anglia and the Midlands.

The hospital has a full range of clinical support departments, including Radiology with MRI, CT Scanning, Nuclear Medicine, comprehensive Pathology services, Anaesthetics, ITU, SCBU and the full range of therapy services, including specialists in rehabilitation and prosthetics.

The radiology department at Stoke Mandeville is equipped for digital subtraction angiography and magnetic resonance angiography.

AmershamHospital

AmershamHospital is situated on the edge of historic AmershamOldTown close to major road links to High Wycombe and Aylesbury and the M25. It is the headquarters for Buckinghamshire Hospitals NHS Trust. The hospital has a specialist inpatient Dermatology Unit, Older Adults Care Unit, Physical Disability Wards, Respite Care and an Outpatient Unit. AmershamHospital has been significantly upgraded as a result of a Private Finance Initiative Scheme (PFI).

WycombeHospital

WycombeHospital is a busy town centre general hospital. It offers a wide range of acute hospital services including 24 hour Accident and Emergency (non-surgical), Critical Care, Elective General Surgery, General Medicine, Elective Trauma and Orthopaedic Surgery and Women’s and Children’s Services. The hospital provides a specialist centre for Urology. Vascular Surgery and Heart Investigative Services are also based on this site.
WycombeHospital has undergone a £40m PFI improvement scheme and is now home to the Buckinghamshire NHS Trust Specialist Surgical Centre. The Centre is a dedicated facility for all routine general surgical, urological and orthopaedic operations that may require a stay of one or more nights.

CONDITIONS OF SERVICE

National Health Service and Conditions of Service for Hospital Medical and Dental Staff (England and Wales) and Conditions of Service for the General Whitley Council.

ANNUAL/BANK HOLIDAY AND STUDY LEAVE

It is the responsibility of all medical staff to liaise with their colleagues and consultants before booking leave and to give adequate warning (at least six weeks). Medical Employment Services must also be informed. External locum cover is not always provided, doctors should check arrangements for their department as detailed in the job description.

NEEDS OF THE SERVICE

Under Para 110 of the Terms and Conditions which were agreed between the Department of Health and the profession, practitioners are required within their own contract and job description to cover, without additional payment, occasional brief absence and then only during the standard working day. Practitioners are also required to perform duties in emergencies without extra payment, provide this is not for a prolonged period or on a regular basis.

Junior Training within the Department

There is a structured teaching programme within the department aiming towards the training of the junior members particularly concentrating on surgical audit. This consists of weekly audit meetings clinical presentations and a monthly academic half day.

Study leave is encouraged up to the maximum allowed periods

Education/Postgraduate Facilities

There are postgraduate medical centres at both Wycombe and AmershamHospitals. The Chiltern Medical Centre is situated on the top floor of Wycombe hospital and is the home of the Chiltern Medical Society. There are excellent library facilities and an active postgraduate programme. The postgraduate centre at AmershamHospital includes a library, conference room and seminar room. There are postgraduate lectures for hospital doctors and GPs at both centres on a weekly basis. Continuing medical education is actively supported.

Public Transport

There is good public transport with both bus and train station within ten minute walk of the hospital.

Sport

Wycombe Sports Centre situated near to the M40 and a fifteen minute walk from WycombeHospital is open to non members and has excellent facilities for many indoor and outdoor activities. It also has a modern fully equipped Health and Fitness Centre. The Chiltern Leisure facility is a similar sporting complex at Amersham. There is Court Garden Leisure Centre at Marlow, which is also open to non-members and offers a variety of sporting activities. There is a fine selection of golf courses in the area and also water sports by the Thames at Marlow.

Leisure

A six screen cinema complex is located just outside the town centre while Wycombe Swan Theatre is a short walk from WycombeHospital.

Banking

None on site but all the major banks are situated in Wycombe town centre – 10 minute walk

Schools

The area is extremely well served with good schools, for both primary and secondary education, including several grammar schools and independent schools.

Local Area

Buckinghamshire is a country of outstanding natural beauty with many places of interest within an hour’s journey of High Wycombe. High Wycombe itself is a centre for the furniture industry, and is situated 50 minutes by train from London. To the west is Oxford and to the south lies Windsor; both are steeped in history and are major tourist attractions. Access to London on the M4, M25, M40, HeathrowAirport and the Midlands is extremely easy.
