Job Title: Primary Care Navigator

Location:Within the Lincolnshire West CCG Localities

Hours of work:35hrs per week

Responsible to:Senior Primary Care Navigator

Salary:£18,797

Job Purpose/Role:

To deliver an integrated model of care in supporting older people, carers and families, working closely with GPs and multi–agency teamsto achieve;

  • Holistic Care Planning and Co-ordination of contact with the individual.
  • A working relationship with the GP Practices in your assigned cluster.
  • A smooth and co-ordinated approach by liaising with a range of multi-disciplinary professionals who are involved in the person’s care.
  • Signposting to a range of services and organisations to promote health, wellbeing and to maintain independence within communities.
  • Provision of information in a wide range of areas relevant to the individual.
  • Reduction in demand on statutory services to combat unnecessary GP appointments, out of hours attendances, hospital admissions and residential care placements.

Main Duties and responsibilities

  • Accept referrals from a wide range of health and social care professionals, along with individuals, their families and carers.
  • Participate in practice level multi-disciplinary meetings.
  • Liaise with a range of multi-disciplinary professionals who are involved in a person’s care ensuring a smooth and coordinated approach.
  • Ensure first contact with the individual is within required timescales.
  • Create a holistic plan of support with an individual, ensuring the plan is copied to the GP and any other relevant professionals involved.
  • Assist individuals to access care assessments and achieve outcomes.
  • Develop, maintain and keep a record ofthe range of local services available and how an individual might access them.
  • Identify if affordability is a barrier to accessing services, and if so provide basic information on relevant benefits and refer for more in depth advice where necessary.
  • Identify unpaid carers and enable access into existing carers support.
  • Keep accurate up to date records of client contact electronically.
  • Deploy volunteers effectively within your cluster to provide ongoing support to Practices.

General responsibilities

  • Work collaboratively with Senior Primary Care Navigator, otherPrimary Care Navigators and Volunteers by attending team meetings and sharing best practice and knowledge.
  • Establish strong links with other Age UK Lincoln staff and contribute to the wider aims and objectives of the organisation.
  • Establish links with the other Age UKs in your area.
  • Work in accordance with Age UK Lincoln’s policies and procedures.
  • Attend any training courses as required.
  • Be prepared to carry out occasional weekend or evening work.
  • To carry out any other duties as may be reasonably required by your Manager.

Essential / Desirable
Qualifications / Health, Social Care level 3 or information and advice background / x
Experience / Experience of working in health, social care or information and advice, in direct contact with people, families and carers in a paid or voluntary capacity / x
Experience of working with multi professional people across health social care the voluntary sector, primary care and GPs / x
Experience of person-centred planning / x
Experience of working with volunteers / x
Skills and Knowledge / Computer literate, able to use standard applications as well as bespoke databases / x
Excellent communication skills both verbal and written / x
Ability to collect data and produce reports when required / x
Can demonstrate an approach to gaining knowledge of local services. / x
Ability to carry out assessments and produce care plans of support / x
Personal Characteristics / Professional and compassionate with ability to be supportive whilst maintaining professional boundaries / x
