Local Recipient Organization Criteria

For an agency to be eligible for funding it must provide all of the following:

Ø  Be non-profit or an agency of government and must have completed local service delivery for a minimum of one year

Ø  Have a Federal employer identification number (FEIN), (note: contact local IRS office for more information on securing FEIN and the necessary form (SS-4) (Web site: www.irs.gov)

Ø  Have a Data Universal Number System (DUNS) number; see the website (http://www.efsp.unitedway.org) for instructions for LRO’s needing to obtain a DUNS number.

Ø  If private, not-for profit, have a voluntary board.

Ø  Conduct an independent annual audit if requesting $25,000 or more in EFSP funds; include audited financial statements for the local agency and if the agency is affiliated with a state or federal organization the audited financials must show Linn County financials. The statement provided must be from the most recent fiscal year.

Ø  Have an accounting system or fiscal agent approved by the Local Board.

Ø  Practice non-discrimination (those agencies with a religious affiliation wishing to participate in the program must not refuse services to an applicant based on religion or require attendance at religious services as a condition of assistance, nor will such groups engage in any religious proselytizing in any program receiving EFSP funds).

Ø  Be cleared of all previous compliance problems form Phase 1 – 33 by the EFSP National Board.

EFSP Requirements Checklist

**Important Note: Applications that do not meet ALL of the program requirements listed or do not submit ALL the necessary documentation by the application deadline will not be accepted or reviewed. THERE WILL BE NO EXCEPTIONS.

Submit the Following:

Ø  An Application for Funding

Ø  A copy of your 501(c)(3) tax-exempt status letter

Ø  A copy of your current volunteer Board of Directors Roster

Ø  Attach a copy of the most recent Board approved budget for applying program(s)

Ø  Attach the organization’s audit or compilation (if requesting $25,000 or more in EFSP funds) for the immediate past fiscal year
