Lions’ 2016 Field Trip Permission Form

When: May 26th

Where: Students will visit the Carnegie Science Center in the morning, then walk to PNC Park to enjoy a Pirates Game. The baseball game is part of the Pirates’ “Education Day” program.

Cost: The fieldtrip has been funded using fudge sales and team dues. If you did not sell fudge or pay the team dues of $25, you must bring in $25 to attend the field trip!

Pick Up: We will NOT be back for regular dismissal in order to watch more of the game. Students will need to be picked up promptly at 4:30! Please do not be late!

What you need to bring: Clothing appropriate for the weather; bagged lunch in all disposable containers (no glass please) or money to purchase lunch at the game; a small drawstring bag to carry items like sunscreen, water, sunglasses, etc.

Phones: Students are not permitted to bring phones or electronic devices to the game. District BYOT policy states phones should be used for educational purposes only, and this is actually supposed to be a non-academic, fun day! We encourage students to socialize and have fun the “old fashioned” way. Similar to a normal school day, student safety and security will be in the hands of teachers and chaperones who are equipped with the resources to contact you and keep your student safe.

REMIND APP: An easy way for us to contact parents about a potentially late arrival would be via the REMIND app. We encourage you to sign up before the fieldtrip if you did not do so at the beginning of the year. To sign up, text “@svlions” to the number 81010 or download the “Remind” App and enter “@svlions” as the class code.

Field Trip Medication Form

School health personnel are generally not in attendance on field trips. Medication and treatments should be administered prior to departure or upon return to school whenever possible. If a student requires life-saving medication (Inhaler/Epipen) or a treatment (Diabetes) in order to participate in the program, and they have permission on file to carry their inhaler or Epipen, they may take it on the field trip with them. If a student requires oral medication to participate in the trip, a parent or nurse will be required to attend the trip. Please complete the following if your child receives a medication or treatment at school on a daily or “as needed” basis that will be required during this field trip.
