2017 Scholarship Application

The Lincoln PTA will award up to an $800.00 scholarship (multiple scholarships with varying dollar amounts may be awarded) to a Cudahy High School senior graduating this year. Selection of the scholarship recipient will be based on meeting the required qualifications, answers to application questions, and review of a submitted essay. The Lincoln Scholarship Committee will choose the scholarship winner.

QUALIFICATIONS:

1.  Applicant must have attended Lincoln Elementary School, Cudahy, for a minimum of three years.

2.  Applicant must be a Cudahy High School senior graduating in June of 2017.

3.  Applicant must be continuing their education at an accredited college, business school or technical school.

Scholarship applications are available in the Cudahy High School Guidance Office or on the Lincoln Elementary homepage. To be eligible, a student must meet the above qualifications and turn in their application portfolio by April 13, 2017. A complete portfolio includes:

1.  Completed Lincoln Alumnus Scholarship Application.

2.  Essay of 500 words or less answering the question, “How did your years at Lincoln School influence your education?”

3.  Photocopy of current grade transcript.

4.  One letter of recommendation from a teacher of your Freshman or Sophomore year.

5.  One letter of recommendation from a teacher of your Junior or Senior year.

All materials need to be sent in one envelope and post-marked by April 13, 2017. All information provided will be treated confidentially and used only by the Scholarship Committee. Application portfolios will not be returned. Applicants will be notified by May 31, 2017.
