Letter to GP practices re iucd/ius & contraceptive implant LCS template

To GP practices signed up for the iucd/ius & contraceptive implant LCS,

We have developed a template that should help with entering data when practices are fitting IUCD/IUS or implants. In addition, provided the procedure page is correctly completed the claims will be collected automatically without needing a quarterly manual submission. Finally by using the data entry page throughout the year, the information necessary for the annual audit will be collected- the sections that are included will be marked: AUDIT. The annual audit aims to ensure practices are following best practice and alert us to high numbers of failed fittings complications.

The template will be used to enter coded data, but the clinician will need to use the EMIS consultation to add further details about the history, examination and procedure.

The template has been developed to be as easy as possible:

History page: we have added prompts so that you can code the history required prior to insertion recommended by the FSRH guidance. In addition the codes are there to record the information and counselling that has been given. There are details of the date of last smear.

Examination page: basic information can be collected during the appointment including weight, height, bmi, blood pressure and pulse.

STD page: it is good practice to do a chlamydia test and this can be recorded on the STD page- this will be collected for the end of year audit and can be used if you are signed up to the sexual health LES.

Procedure page: THIS PAGE CONTAINS THE SECTIONS THAT WILL COLLECT THE DATA FOR PAYMENT- PLEASE READ THE INSTRUCTIONS VERY CAREFULLY.

-IUCD fitting- both IUCD fitting- enhanced service completed and IUCD fitted

-IUCD 6 week check-both IUCD 6 week check-enhanced service completed and IUCD check

-IUCD removal-both IUCD removal –enhanced service completed and IUCD removed

-IUS fitting- both IUS fitting- enhanced service completed and Mirena coil inserted

-IUS 6 week check- both IUS check-enhanced service completed and Mirena coil check done

-IUS removal- both IUS removal –enhanced service completed and Removal of Mirena coil

-Contraceptive implant insertion- both Implant fitting- enhanced service completed and Implant insertion

-Contraceptive implant removal- both Implant removal- enhanced services completed and Implant removed

Data entry and Audit page: there are sections here to record the reason for fitting, complications or failed fittings for IUD and implant problems.

Please remember procedures done by a third party cannot be claimed for- the searches ensure that these will not be claimed for inadvertently. However there is a clearly marked section in the data entry page which allows coders or clinicians to add data from third parties, for example from clinical letters.

At the end of the document are links to the latest NICE guidance and clinical guidance from the FSRH.

Please start using the template on 1st April 2015 and then the payments for quarter one of 2015/2016 will be possible.

Throughout the year please encourage all clinicians to add to the data page and then we can collect the information for the annual quality monitoring.

We hope to provide an additional search that will link the procedure to the clinician so that we will be able to have numbers of insertions per 12 months. As you know the FSRH require a minimum level of 12 insertions in 12 months for IUCD/IUS fittings and we will be monitoring this on an annual basis. Practices will be contacted if they do not match or exceed the minimum levels expected.
