LETTER OF INTENT TO PARTICIPATE IN COLLEGE CREDIT PLUS

Having been informed by Canton South staff in a group meeting and/or individual meeting and having received appropriate and adequate counseling regarding the CCP option as called by law, we the undersigned, chose to do the following:
I wish to participate in CCP classes for the 2016/17 school year at Canton South High School.
ü I understand that it is the decision of the college to accept/deny my application
ü I understand that if I take these courses for college credit, I will begin my college transcript
ü I understand that I may be held financially responsible if I drop a CCP after the 5th day of class or if I fail the course
ü If I drop a CCP course after dates established by the college, I will receive a W (withdrawn) or a WF (withdrawn/fail) on my high school and college transcript, and the WF will be calculated into my grade point average
ü I understand if I do not turn in a completed CCP enrollment packet by March 11th, 2016, I will not be eligible to take CSHS CCP courses for the 2016/17 school year
ü I understand that if I can’t Compass Test on the available dates, I will need to schedule a testing appointment with Stark State College
By signing this form, you are not required to participate in the coming school year; this is just a notice to inform the school that you are interested and are aware of the CCP procedures. After you have scheduled for CCP classes, please notify your counselor ASAP if you choose not to follow through with taking CCP courses for next school year.

In order to participate in CCP courses at CSHS, student must submit packet by
March 11th, 2016.
In order to participate in CCP classes on a college campus, an intent form must be returned by Friday, April 1, 2016.
