Information, Releases and Agreement (“Agreement”)
I am the parent or legal guardian of the Player named below and hereby agree to all of the following, including without limitation all permissions, releases and authorizations, and further agree that any or all of the information found here may be included in any roster, book, website or publication of the Beaverton School District, Sunset High School, Sunset Football (all of whom are hereinafter referred to jointly and severally as “Sunset”) and any league in which Sunset High School participates.

Register beforeMay 30th for BOTH Kickoff AND Apollo Camps and SAVE $50 per player.
If this Agreement is completed, returned and the $175 fee is paid in full on or before May 30th, 2017.
After May 30tht, the fee is $225 per player for both camps.
Note: There is not a reduced rate for more than one player per family. There is no refund or rebate if you
cannot participate in the either camp and your registration is not assignable. You should complete a separate Agreement for each Player.
Financial Need : Sunset Football is committed to having every interested student athlete participate in these camps. There are options available directly from Coach Merrick and an agreement must be worked out prior to May 30th.

Kickoff Camp Dates/Info: (9 Days Total) Kickoff Camp will be held from 2:45pm to 5:00pm (Incoming Frosh 4:30-6:00) at Sunset High School June 1st/2nd and 5th-9th. Saturday June 3rd camp will be from 10:00-1:00. Kickoff Camp will conclude on June 10th at Canby High School in Canby, Oregon. The Registration Fee includes transportation from Sunset High School to Canby High School on June 10th and lunch/dinner on June 10th only.
All necessary equipment/gear will be issued after school Wednesday May 31st. Participants will receive a T-Shirt, Shorts and a practice jersey all of which are theirs to keep.
Apollo Camp Dates/Info: (9 days total) Apollo Camp will be held in the afternoons/evenings at Sunset High
School, July 31st-August 4th and August 7th- August 10th.
PERMISSION AND RELEASE: I, hereby give unconditional permission for the Player to
participate in any and all activities sponsored by or through Sunset, including without
limitation Kickoff Camp, Apollo Camp and any and all camps, conditioning, practices, games,
scrimmages, transportation, parties, award ceremonies and other functions (all of which are
hereinafter “Activities”). Further, for myself, and for any spouse (or other guardian), and for
the Player, I hereby fully, finally and unconditionally waive, release and discharge any and all
claims and damages, whether direct, indirect, general, special, reliance, incidental,
consequential, exemplary or any otherwise, whether in an action based on contract, tort,
negligence (active or passive), strict tort liability or otherwise. (including but not limited to
claims and damages for injury, illness or death) whether known or unknown, now existing or
arising at any time in the future against, the Kickoff Camp Director, the Apollo Camp
Director, Beaverton School District, Sunset High School, Sunset Football, Canby School
District, Canby High School, Tualatin Hills Park and Recreation District, all participant teams,
districts and schools at the Kickoff Camp or Apollo Camp, any and all persons and entities
transporting the Player (All of the persons and entities in italic, bold and underlined above
are hereinafter referred to as “PERSONS’ AND ENTITIES’) or any of them, as well as any and
all of the PERSONS AND ENTITIES’ employees, officers, directors, organizers, sponsors,
supervisors, leaders, participants, officials, referees, volunteers, coaches, sponsors, other
players, agents and representatives which arise by reason of or are in any way related to
this Agreement or the Activities.
Medical Authorization: In addition, I hereby authorize the Kickoff Camp Director or the
Apollo Camp Director and in his absence his designee, to act for me in any emergency
situation requiring medical transportation, treatment or attention and to authorize any or all
of the same.
Severability. Nothing contained herein shall be construed so as to require the commission
of any act contrary to law, and wherever there is any conflict between any provisions
contained herein and any present or future statute, law, ordinance or regulation the
provision of this Agreement which is affected shall be curtailed and limited only to the
extent necessary to bring it within the requirements of the law.
