Job Title: Key Worker (Stockport Targeted Prevention Alliance)
Salary range: £19,000 per annum
Hours: 37 per week
Duration of post:Funded until September 2018, with possibility of extension
Location:A central Stockport office with regular travel across Stockport will be required
Responsible to:The Senior Key Worker
Responsible for:Assisting in the delivery of a person-centred health and wellbeing service, improving outcomes for Stockport people and preventing or delaying entry into statutory services.

Main purpose of the post
- To work positively as part of the Stockport Targeted Prevention Alliance team to facilitate, support and co-ordinate access to information, support, groups and services that will provide residents with a quality experience from the start and throughout their contact with the Alliance
Main tasks and responsibilities
Service delivery
- To provide information and advice to peopleover the telephone and through face-to-face support. This may be in a town centre or community location, or home visit if required
- To consult with, and support,people to enable identification of the person’s needs, showing empathy and communicating clearly in a way people can understand
- To provide appropriate referral, signposting and support and follow up support to each person as appropriate within the remit of the Alliance
- To work with the Operations Manager, Senior Key Worker and other key workers to develop a work programme, including outreach activities
- To act as a champion for the overarching aims of the Alliance and its member organisations
- To work as part of a team delivering a first class, professional customer service, using a range of communication methods, to ensure that where possible customer enquiries/complaints are resolved at the first point of contact
- To use sound judgement and take decisions within established procedures for each enquiry,including recording, processing, monitoring and adhering to agreed Key Performance Indicators and quality standards to maximise customer satisfaction
- To attend any appropriate training as required
Administration
- To support effective qualitative and quantitative monitoring and evaluation of the service by collecting and recording information and data as required
- To assist with maintaining appropriate administrative systems and records to ensure effective operation of the Alliance
- To assist with the effective promotion of the work of the Alliance through voluntary and community sector networks, health and social care services and others as required, creating comprehensive coverage across the Borough
- Attend and participate in relevant conferences meetings and training events or other appropriate meetings as required in line with the scope of the post
Other
- To positively represent the Stockport Targeted Prevention Alliance in Stockport at all times
- To manage and prioritise own workload in agreement with the Senior Key Worker and to work positively to ensure the smooth and effective delivery of the project
- To carry out such other duties as the Senior Key Worker, Operations Manager or other Alliance managers may reasonably require in relation to the post
Key WorkerPerson Specification
Qualifications / Essential / Desirable- Have attained a good level of education
- Educated to degree level or equivalent
- Evidence of motivation for personal development
Knowledge / Essential / Desirable
- A sound understanding of voluntary & community sector
- An understanding of the principles of I&A provision
- A broad understanding of issues relating to adult health and social care
- An understanding of the voluntary and community sector and / or information and advice in Stockport
- An understanding of the importance of confidentiality
Skills and experience / Essential / Desirable
- Proven ability to communicate effectively, both in writing and verbally, with people at all levels.
- Proven ability to build effective working relationships
- Confident in use of IT and web based information
- Experience of delivering information and/or advice to the general public
- Experience of working within Priority 1 areas
- Promotional skills and experience
