Job Title : Care Coordinator, Personal Assistance Services
Position Description
I. Position: Care Coordinator, Personal Assistance Services (RN), (Direct Service Exempt)
II. Reports To: Assistant Program Manager, Personal Assistance Services
III. Position Summary: To provide timely and efficient services to the PAS program by visiting consumer homes and assessing and developing appropriate plans of care; act as liaison to physician, consumer and agency; and provide direction to and evaluate performance of personal care workers.
IV. Essential Functions:
A. Evaluate the need for service and make referrals to other services as appropriate.
B. Secure written orders from the recipient's physician. These orders are to be renewed annually unless the physician or payment source specifies otherwise.
C. Provide payment source with necessary documentation to approve and maintain service.
D. Maintain proper medical and service records. Monitor time and service reporting mechanisms for PCW's and instruct on their use.
E. Develop a plan of care for the consumer giving full consideration to the consumer's preferences for service arrangements and choice of Personal Care Workers. Include a copy of the care plan in the consumer's service record, and review the plan at least every 60 days, updating as necessary.
F. Give the worker written instruction about the services to be performed and demonstrate to the worker how to perform the services.
G. Evaluate the competency of the PCW to perform the services.
H. Assist in developing and implementing training programs for PCW's.
I. Attend training sessions and conferences as needed.
J. Coordinate schedules and manage services for consumers with Consumer Service Representatives.
K. Be available for on-call services on a rotating basis.
L. Provide advocacy and support for the Independent Living Philosophy to all PAS staff and consumers.
IV. Qualifications:
A. RN degree and current state license to practice nursing.
B. Experience in home health, rehabilitation, public health, and/or independent living.
C. Good written and verbal communication skills.
D. Experience with/knowledge of Medicaid system.
E. Transportationally mobile or able to arrange own transportation.
F. Knowledge of federal, state and local laws related to community based health care services.
