Job Title:Wellbeing Participation Coordinator

Office Base: Hartford (with travel across Cheshire West and Chester)

Primary Purpose:

The Wellbeing Participation Coordinator will be responsible for promoting innovative ways to engage and develop case work based placement/volunteer opportunities that contribute to the sustainable delivery of the Wellbeing Coordinator service. The post holder will support the management, recruitment and training of volunteers and explore new ways to increase volunteering and social action.

Main Duties:

  1. To promote volunteering opportunities in a variety of cost effective and creative ways in order to attract appropriate numbers of volunteers/placements to the scheme.
  1. To be responsible for the administrative tasks of the scheme, and keeping written records and statistics according to set guidelines.
  1. Liaising with the SWBC and be responsible for the recruitment, selection, interviewing, inductionand ongoing support of volunteers.
  1. To be responsible for the supervision and support of the volunteers. Including addressing any concerns which are raised by the volunteers about their client and for seeking resolutions, if appropriate/possible, working with the wider WBC team.
  1. To be responsible for coordinating potential referrals/caseloads from the Wellbeing Coordinators and for allocating them to appropriate volunteers.
  2. To liaise with WBC’s to ensure that all monitoring data required by Commissioners is kept up to date and to provide regular reports on request to the Senior Wellbeing Coordinator and Head of Services.
  1. To monitor, review and maintain the qualityof the service given by the volunteers in line with AUKC expectations and policies.
  1. Liaising with the WBC teams to promote the service in the community and to other care professionals.
  1. To review training provision/requirements for the volunteersand looking at innovative ways to approach this in a cost effective way.
  1. To develop robust volunteer communication pathways - holding regular volunteer meetings and integrating them into WBC team meetings to ensure inter- team partnership communication arrangements are working.
  1. To ensure all CPD records are updated for volunteers/placements as required.
  1. To ensure that volunteers follow Age UK Cheshire’s policies and procedures including Lone Working, Data Protection and Confidentiality and safeguarding.
  1. To ensure that the concept of Wellbeing is promoted to a wide audience in the West Cheshire CCG area.
  1. To target individuals living in the community who have long term health conditions and to promote preventative messages through a variety of innovative means, including social media.
  1. To work with partners in the CCG and Local Authority on delivering appropriate health messages to the community.
  1. To attend events and provide talks to a variety of stakeholders and partners to promote the service.
  1. To report to the line manager on a regular basis and to produce written reports for the Trustees of Age UK Cheshire and for the commissioners of the service as required.
  1. To attend staff meetings, supervision and training sessions as required.
  1. To work to the standards required by Age UK Cheshire and to follow their policies and procedures. To have particular regard to Health and Safety requirements of the volunteers.
  1. To undertake a share of the administrative tasks in the office and to assist in the smooth running of that office when other members of staff are unavailable or on holiday.
  1. To assist in fundraising activities organised by Age UK Cheshire.
  1. To undertake such duties as may from time to time be reasonably requested by the Chief Executive Officer or Senior Management.

PERSON SPECIFICATION

Essential Criteria:

  • Experience of working with and managing volunteers i.e. recruitment, selection, support and training
  • Good organisational skills, time management and ability to work on own initiative and as part of a team
  • An understanding of equality and diversity issues as they impact on people of all ages over 18
  • Ability to work with a wide variety of people from the statutory, voluntary and private sectors
  • Ability to set and manage priorities within a busy environment
  • IT skills, including Microsoft Office suite, spread sheets, internet, email, databases and word processing
  • A good understanding and application of Data Protection and Confidentiality
  • Experience of supporting the promotion of services
  • A commitment to empowering people to exercise their choice and control over the way they live their lives
  • Ability to travel freely across Cheshire

Desirable:

  • A sound knowledge and understanding of the issues and concerns that can affect the lives of older people 55+ and/or people between the age of 18-55
  • Experience of running training events/meetings
  • Experience of giving talks in public
  • Knowledge of the care and support services available in the local community
  • Use of a vehicle for work related purposes

Wellbeing Participation Coordinator – Supplementary Information

Age UK Cheshire was formed in 1983 and we have been providing services for older people in the Cheshire area since this time.

The Wellbeing Coordinator Service (WBC) at Age UK Cheshire is an initiative working in partnership with the CCG, GP practices and integrated health teams. WBC teams are based in GP practices in a non-clinical role offering practical solution based support for patients identified with unmet social needs which impact on their health and wellbeing. The service supports people over the age of 18, living with a long term health condition and who are at greater risk of admission to hospital and /or escalating visits to GP or care services due to the social issues they face.

As part of the development of the service we are looking for a Wellbeing Participation Coordinator who will be responsible for promoting innovative ways to engage and develop case work based placement/volunteer opportunities that contribute to the sustainable delivery of the Wellbeing Coordinator service. The post holder will support the management, recruitment and training of volunteers and for exploring new ways to increase volunteering and social action.

The Wellbeing Service provides the option for patients with unmet social needs to be referred to a non-clinical member of the GP practice team who will signpost and provide the person with the information and support they require in order to reduce future reliance on health services and to support them to remain independent in their own homes in the community. The service aims in particular to support those patients who live with long term health conditions and are at greater risk of admission to hospital, excess GP visits or care services.

The service supports individuals to build personal resilience in self-managing their health and wellbeing through assessment and review of individual needs, development of individual wellbeing plans, and motivating behavioural change. There is a wealth of literature and evidence regarding the use of brief interventions, motivational interviewing, and health coaching and the pilot is informed by NICE Guidance and Quality Standards and Pathways. The evidence shows improved outcomes in physical health, mental and emotional wellbeing, and reduced demand on health and social care services.

Location: Hartford, Northwich with travel across Cheshire West and Chester.

Hours: 25 hours per week (to be worked over 4 / 5 days with some flexibility at recruitment stage at how these hours can be established, which will be discussed at interview). Occasional evening or weekend work to support organisational promotional events or service delivery will be required.

Salary: £14,079 per annum.

Travel: The nature of the job will require travel and candidates must be able to meet this requirement. Travelling expenses will be paid for travel incurred in the course of duty (mileage paid at Inland Revenue dispensation rate – currently 45p per mile).

Holiday Entitlement: Five weeks leave p.a. (excluding Bank Holidays) rising after 5 years continuous service by one day per year (pro rata) to a maximum of 6 weeks leave after 10 years.

Contract: All new staff are subject to a six month probationary period. Four weeks’ notice is required to terminate employment by either side i.e. the employee or Age UK Cheshire. During the probationary period, one week’s notice is required by either side.

Funding: The project is funded by West Cheshire Clinical Commissioning Group and is in place until March 2017. Extension beyond this is subject to further funding agreements.

Pension: Age UK Cheshire has an automatic enrolment workplace pension scheme in place for eligible employees.

Health Care: After the 6 months probationary period staff will automatically join the Age UK Cheshire’s Health Care Cash Plan and will be entitled to receive healthcare benefits.

Childcare Vouchers: After the 6 months probationary period staff will have the option to make use of the Age UK Cheshire Childcare Voucher scheme (available to eligible staff whilst in receipt of salary).
