Job Title: TLP/RRH Client Coordinator

Reports To: Sr. Program Director

General Purpose:

Coordinate client services for young adults within the Transitional Living Program and transition clients into Rapid Re-Housing Program at City House for ages 18 to 22 yrs old who have been effected by abuse, neglect, and homelessness. Working as part of the leadership team of TLP and RRH, reporting to the Sr. Program Director, and participating in the on-call rotation.

Essential Functions:

1.  Participates as a member of the Interdisciplinary Team to meet the physical, emotional, educational, developmental and social needs of the young adults, as outlined in each clients plan of service, medical reports, psychological evaluations and developmental assessments and through staffings. Participation includes but is not limited to staffings, in-service trainings or other forms of communication.

a.  Works directly with TLP Case Manager in determening and reviewing clients program goals including but not limited to physical, emotional, academic, employment, life skills, and community goals.

b.  Assists with TLP & RRH client intakes and admission assessment

2.  Gather data necessary to produce outcomes for both TLP and RRH client’s goals while conducting client check-ins.

3.  General understanding to maintain program policies and guidelines for both TLP and RRH clients

4.  Provides referrals and recourse for young adults as needed related to services plans.

5.  General understanding of TLP resources and referrals such as parenting support, college financial assistance, health care, SNAP, other resources

6.  Set up all Client Check-In binders

  1. Create binder for all new TLP clients
  2. Meet weekly for client check-ins and maintain client check-in binder weekly including (client hours, strikes/write-up/weekly goals/other)
  3. Pass requests

7.  Participate with Case Manager in 6-week Service Plan reviews with TLP clients

8.  Assist Case Manager maintaining all TLP client files and appropriate documentation

9.  Develop and maintain RRH client files and documentation per grant requirements

10.  Client Appointments

  1. Assisting and/or accompany clients in scheduling/attending general appointments related to weekly goal setting
  2. Gather information from staff and client to review during check-ins

11.  Assist in entering any TLP residential and non-residential clients into HMIS

  1. Enter and maintain all reports for RRH Clients within HMIS

12.  Participate in client staffing by sharing clients progress towards goals

13.  Maintain open communication with other program staff including TLP House Managers and TLP Operations Manager regarding TLP client’s goals and needs

14.  Prepares clients and staff for client’s discharge

  1. Assisting TLP Case Manager with any documentation updates for TLP client and TLP records
  2. Lead transition of any TLP client into RRH program

15.  Participate in on-call rotation responding to afterhours crisis related to all agency programs and referrals.

16.  Sets high standards for professional, technical, personal conduct and competence and is experienced/skilled working with Excel, Word, Powerpoint, and Database entry

17.  Reports immediately any suspected child abuse/neglect of a child.

18.  Emphasizes safety to keep employee injury to a minimum.

19.  Maintain full confidentiality regarding residents/clients, other employees and agency issues. Employee will not discuss agency business or difficulties when residents or clients are present.

Minimum Physical Requirements:

§  Ability to use up to 50 pounds of force occasionally, and/or up to 20 pounds of force frequently and/or up to 10 pounds of force continuously to move objects and/or people.

§  Good depth perception and field of vision required.

§  Ability to bend, stoop, twist, turn, reach, lift, carry, pull, climb, and kneel, with approximately 50% of the time spent walking and standing.

§  Ability to recognize differences in sound, such as voices/noises that are loud and playful instead of angry and combative, and the ability to differentiate tone and volumes in conversations.

Qualifications/Basic Job Requirements:

·  Bachelor's degree in social work or related social service field.

·  Minimum of one year of experience working with at-risk and/or homeless youth or young adults or related experience.

·  Knowledge of issues related to youth/young adult homelessness, abuse and neglect, life skills, child development and child and adolescent development.

·  Valid driver's license and insurance.

·  CPR Certification must be current, or obtained within 30 days of hire. Willingness to take first aid training and restraint training.

·  Effective communication skills, both oral and written. Effective organizational skills. Ability to establish and maintain effective working relationships with other employees and the public. Ability to perform duties with minimal supervision if necessary.

·  This position is funded through state and federal grants and is subject to termination if funding ceases.
