President’s MESSAGE

SD ANFP Members,

I’m very excited to have Bev as our Newsletter Editor!

Be sure to let her know if there is something you’d like on the website. If any members are interested in volunteering to be the President-Elect, please let any board member know. If you have questions, we will get answers to you.

I have volunteered to be secretary a couple of times and have learned many new things (I’m still working on the computer/technology stuff). Meeting other people around the state has been great.

I live in the country near Rockham. My husband raises cattle and crops. The sheep were sold in the last couple of years. We have two silly cats who keep us entertained. They like water so are always in the bathtub hoping we’ll turn the faucet on a little. One likes to play fetch with a paper ball.

I love to read, watch birds, garden and have learned to weave. At Head Start, I make menus, train cooks and work with our Health Coordinator on child health requirements.

SD ANFP provides such great training we can use in our careers. The Spring SD ANFP Conference will be held in Watertown April 22-24. Find the scholarship form on the website to assist with conference expenses. See you then!

Membership Chair Report by Dawn Strand

As of Feb 4, 2015 we have 151 members with 143 of these being certified, leaving 8 not certified.

We have 2 new members since our fall meeting to report. Tommi McLaughlin of Isabel, SD a Pre-Professional Member and Rosa Jundt of Eureka, SD a Professional Member. Welcome Tommie and Rosa! We have no new student members at this time.

I’m Back!!!! Just when you think you are done with me!!!!! If you don’t know who I am, I’m Linda Stewart and I will be serving as your Treasure for the next 2 years. In the past I have served as Government Affairs then went onto President Elect, President and then Past President. I’m so honored that you have voted me to be on the board again as I feel this is such an important organization to be a part of. We have come a long way in this field and we are starting to be recognized for it.

Just a bit about myself, I have been a CDM since 2004 and have worked in long term care pretty much all my life as a CNA, CMA & CDM-CFPP I lost my parents at a very young age and I have made it my life goal to take care of the older generation. I love them and have learned so much from this awesome generation. Their stories of the days gone by are what I live for. I loved making their favorite recipes for all to share. Now I am working out of the kitchen but not far at an Assisted Living/Memory Care facility as a Business Office Director. I feel like I have been blessed to work full circle with every aspect of the resident’s life.

As of today we have $9073.93 in checking and $2260.41 in savings.

Secretary Report by Lynette Shattuck

My name is Lynnette Shattuck, I am from Mitchell SD. I have been in some type of Food service for most of my life. I took my Dietary certification in 1990 and then went on and got my nursing degree in 1993. I have worked in both hospital and nursing home settings. I am currently working at Avera Queen of Peace hospital in Mitchell SD as the Nutrition and Meals on Wheels Coordinator. This allows me to utilize both my Dietary and Nursing license. I have a wonderful husband, Steve, who is the owner/manager of Independent Viking Glass here in town. We have two great kids, Cory, who is an electrical engineer living in Denver and Alisha who works for the postal service here in Mitchell. And we have one grandson, Jaxton, who just turned one year in January and he is the sweetest little boy ever! I enjoy warmer weather, golfing, bowling and hanging out with family and friends.

In the January 2015 issue of Nutrition & Foodservice Edge publication, there is 1 hour of CE credit you can earn by reading and completing the review questions on the article titled Documenting in the Medical Record. You can complete the quiz online by purchasing the quiz in the ANFP Marketplace. Visit www.anfponline.org/marketplace, selectPublication”, then select “CE Article” at left then search the title“Documenting in the Medical Record” and purchase the article.

Also, in the same issue, you can also earn another CE for 1 hour by reading and completing the article “Building a Great Staff on a Limited Budget”. Follow the above instructions to purchase through the ANFP Marketplace. Click on the article “Building a Great Staff on a Limited Budget” and purchase the article.

There are numerous other CE’s you can purchase by visiting the ANFP Marketplace. Go to www.anfponline.org/marketplace and click on the CE that would interest you and purchase.

Remember we have to report our own CE’s now on the ANFP website. Go to and go to member log on and enter your username and password. Once you have entered this information, click on My Certification on the left hand side of the column. This will take you to your own personal certification page which shows how many total hours you have and how many hours you need yet till the end of your certification cycle. To enter a CE from the article you have purchased you then click on Report CE. Find the title Authoring and click on the Report Activity button next to it.It will then ask you for the article title, publication name, publisher, publish date, and the general hours or the sanitation hours. Once this is all filled out you can click on report button or if you have another CE to report, you click on save & add another button. . Once you are done entering your CE’s, you click on report.

Getting Acquainted with your Newsletter Editor- Bev Vogt, DTR, CDM, CFPP

I started my career in dietary in 2001 as a cook at the Douglas County Memorial Hospital in Armour. In 2003, I passed my credentialing examination for the dietary manager to become a CDM. I continued on with my education and took on line classes thru the University of Minnesota and obtained my Associate in Applied Science to become a dietitian technician. I graduated in December of 2005 and passed an exam to become a registered dietitian technician. In 2007, I was awarded the Outstanding Dietetic Technican of the Year by the SD ADA. I am currently still working as a dietary manager at the same facility. We do have an assisted living connected to our facility also. We cook for the Senior Citizens and do the home delivery meals for the community of Armour. I have 5 employees who do a fantastic job.

I am the mother of four children and also the grandmother to six grandchildren. Two of my children live in Rapid City so I make frequent trips out there and do a lot of facetiming with my grandchildren. My husband works as a lead highway worker for the SDDOT out of Armour. For fun, we like to go to Nascar races, camp with our family and just hang out with the children and grandchildren. We recently took a trip to Cancun for the first time over Christmas, which I loved every minute of the warm weather.

I love working with our patients and teaching them about their diets and just visiting with them. They each have a different story to tell. Each day is a new day and we never know what that day will bring. Live each day to the fullest and enjoy every minute of every day! I look forward to meeting each and every one of you at our meetings and I will do the best of my ability as newsletter editor.

ANFP Practice Standards: Documenting in the Medical Record

These guidelines offer help for CDMs responsible for documenting nutritional status and progress with therapeutic diets. Refer to them as needed.

Professional Standards of Practice serve as the basis for quality dietetic practice for dietary managers. The standards that follow—updated in December 2014—provide guidelines for certified dietary managers to use when documenting food and fluid intake in the medical record.

Standard 1:

The certified dietary manager, certified food protection professional (CDM, CFPP) shall ensure that nutritional screening data is accurately obtained and recorded in the medical record in accordance with state and federal regulations and facility policy. The CDM, CFPP shall ensure that food and fluid-related interventions are added to client care plans..

Criteria

1.1 A policy is in place stating which qualified interdisciplinary team (IDT) member will ask client interview questions regarding factors influencing nutritional status. The policy will include how other family members or surrogate decision makers are to be included in the interview and care planning process and how this information is communicated to the IDT.

1.2 CDM, CFPPs will receive training to obtain nutritional screening information.

1.3 Nutritional screening data is obtained from client interviews and/or from surrogate decision makers in a timely manner that complies with facility policy and regulatory guidelines.

1.4 Nutritional screening includes documentation of food and fluid intake in the medical record.

1.5 Identification of nutritional problems are included in screening and documentation within the medical record. Examples of nutritional problems:

1) Client consistently leaves greater than 25 percent of meals uneaten,
2) Client does not consume adequate fluids on a daily basis,
3) Client does not have ability to chew, has mouth pain, or has signs and symptoms of a swallowing disorder,
4) Client has difficulty using regular feeding utensils or dinnerware,
5) Client is unable to feed self,
6) Usual dining routines, behaviors and appetite have significantly changed,
7) Client has experienced a significant change in weight.

1.6 Progress notes will reflect observation of a client’s response to the dining environment (does the environment stimulate client to consume food and fluids and socialize with peers, etc.) and dining preferences.

1.7 Progress notes will reflect observation of a client’s food and fluid intake and usual dining routines and behaviors.

1.8 Nutritional interventions are care planned with appropriate participation of IDT members to address family, staff, or client’s nutritional concerns.

1.9 CDM, CFPPs collaborate with the dietitian* and client in establishing goals and approaches for a client’s nutritional care plan that are routinely monitored, reviewed, and revised.

1.10 CDM, CFPPs use standardized forms for documenting data obtained from the nutritional screen (such as components of the Resident Assessment Instrument (RAI): Minimum Data Set (MDS), Care Area Assessment (CAA), and care plan).

1.11 All medical records are legal documents. With use of an electronic health record (EHR) system, electronic signatures are acceptable. Following are guidelines for an EHR system:

1.11.1 The healthcare facility has a written protocol describing the attestation policy in force at the facility.
1.11.2 Each person responsible for an attestation has an individual identifier.
1.11.3 The EHR has built-in safeguards to minimize the possibility of fraud and HIPAA violation.
1.11.4 The date and time is recorded from the computer’s internal clock at the time of entry.
1.11.5 The EHR system controls what actions and areas an individual can access or enter data, based on the individual’s personal identifier.

1.12 In cases where facilities use a paper medical record, entries will be in black ink, dated, signed with full name and title, and never backdated or erased. Medical record errors are connected by a one-line strike out, initialed, dated, and labeled “error”.
