JOB TITLE: Health Care Assistant

GRADE: AMC Grade 3

RESPONSIBLE TO: Practice Nurses and Practice Manager

DUTIES & RESPONSIBILITIES OF THE POST:

1.  General Clinical Duties

a.  Giving health education advice and support about smoking, alcohol, exercise, diet, sun and safe sexual practices. Encouraging patients to attend any of the screening programmes available to them, referring to structured education programmes and formulating patient action plans.

b.  Simple dressings that have already been seen by the registered nurse, following the prescribed treatment regime

c.  Repeat blood pressure monitoring in accordance with protocol

d.  ECGs when requested by Health Care Professional

e.  Spirometry when requested by Health Care Professional

f.  Base line measurements such as height, weight, BP, urinalysis, waist circumference, BMI’s, visual acuity and pulse oximetry.

g.  New patient medicals, CVD health screening, well woman/man checks, learning disability checks referring on anything outside of scope or expertise.

h.  Long term condition monitoring and formulating patient action plans.

i.  Checking and teaching inhaler technique

j.  Phlebotomy

k.  Assisting at vaccination clinics, giving seasonal influenza & Pneumococcal polysaccharide and documenting what has been given in accordance with SOP.

l.  Removal of simple sutures & clips

m.  Ear syringing/aural care.

n.  Assisting with and chaperoning minor ops, general surgeon outpatient clinics, coil and implant fitting and removal.

2.  Nursing Administration

a.  Stock control and ordering

b.  Monitoring and control of vaccine fridges

c.  Maintenance of clinical equipment and planning and organising of regular servicing

d.  Keeping work area clean and tidy in accordance with infection control policies

e.  Organisation of clinical and sharps waste storage and disposal

f.  Awareness of and compliance with requirements for Health and Safety and infection control

g.  Assisting in auditing of practice nurse work

3.  Communication

a.  Liaison with all members of multidisciplinary team

b.  Strict adherence to the principles and procedures surrounding confidentiality

c.  Chaperoning of patients as appropriate.

d.  Polite and effective communication with patients

e.  Prompt attention to electronic communications via email, clinical system (SystmOne) tasks and Intradoc with timely response and actions.

4.  Personal development

a.  Completion of NVQ level 3 in Care

b.  Development and maintenance of a personal learning plan

c.  Participation in the Practice Appraisal system

d.  Attendance at monthly nursing team meetings and other training events considered appropriate for the post

e.  Mandatory Training

5.  Any other delegated duties considered appropriate for the post

SPECIAL REQUIREMENTS OF THE POST

·  The HCA will at all times work under the direction and supervision of a Registered General Nurse or GP.

·  The HCA will at all times conduct themselves in a professional manner, which is befitting of a health care worker.

·  The HCA must be aware of their level of competence and practice within it. They should only carry out work which they have been given instruction, or training in, and feel confident and competent to carry out.

·  All patient consultations will be documented on the patient’s computer record

·  Patient confidentiality and Information Governance guidelines must be adhered to at all times

·  The HCA will assist the nursing team in delivering a high standard of care, which is evidence based and in line with surgery protocols
