Job Title: CRA Team Leader

Position Overview

The Team leader provides assistance to the Senior Residence Counselor in the day to day operations of the Community Residence, in addition to providing direct services to residents.

Reporting Relationships

The Team Leader reports directly to the Senior Residence Counselor

Duties and Responsibilities

  • Ensures that sites and charts are in audit ready condition at all times.
  • Implement Agency medication administration policy and procedure
  • Implement fire safety procedures including monthly, unannounced fire drills
  • Review daily notes on a routine and regular basis
  • Promptly report any identified problems to the Director of Residential Services
  • Assure that the rights and dignity of residents are maintained at all times; assure that resident

grievances are processed in a timely manner

  • Assists the Senior Residence Counselor in the develop and implementation of Individual Service Plans in cooperation with direct care staff; assist staff in working with residents toward identified goals; regularly and routinely document resident progress toward goals in accordance with Agency policy
  • Works one to one or in groups to provide services to residents
  • Assist with management of resident discharges; complete and submit related paperwork, as directed
  • Develop and maintain effective working relationships between and among staff and other service providers
  • Assist in assuring timely reporting and review of untoward incidents and events; participate on Agency Untoward Incident Special Review Committee
  • Initiate emergency procedures when indicated, including utilization of police, Crisis Services

and other emergency personnel, as appropriate

  • Assists with the planning and implementation of resident activities, in cooperation with residents and direct care staff
  • Provides coaching and training to staff related to service delivery, as directed by the Senior Residence Counselor
  • Identifies any deficiencies in record keeping practices
  • Perform other duties as assigned

Requirements

  • Qualified Mental Health Professional (QMHP) with Bachelor’s degree in Social Work, Human Services or related field plus one (1) year paid experience working with individuals with handicapping disabilities; or, Associate's degree in Human Services plus three (3) years’ experience as above; or, High School diploma plus five (5) years’ experience as above
  • Able to demonstrate at least (1) one year of paid leadership experience
  • Evidence of commitment, sensitivity and creativity in working with and for individuals with handicapping disabilities
  • Experience in developing service plans and conducting assessments
  • Must have a flexible schedule
  • NYS Driver's license with evidence of good driving record and reliable transportation
  • Must have use of a reliable automobile
  • Must undergo and pass a criminal background check
