JOB DESCRIPTION:

The Continuum of Care Coordinator will work directly with the Executive Director of the Tarrant County Homeless Coalitionand homeless service provider professionals to assist with the planning, facilitation, implementation,and program evaluationto support effective coordination, collaboration and delivery of the Continuum of Care, the community-based homeless assistance program planning networks. The Coordinator will assist in the administration of the CoC Program, operation of the CoC, and agency support for HUD reporting from the HMIS system.

ABOUT TCHC:

The Tarrant County Homeless Coalition leads, coordinates and develops strategies and resources to end homelessness. TCHC is a non-profit agency serving member organizations and the community. As recognized by the U.S. Department of Housing and Urban Development, TCHC serves as the lead agency for the Continuum of Care responsible for year round planning, evaluation, and monitoring of the systems of care provided to persons at risk of, or experiencing homelessness in Tarrant and Parker counties. TCHC is also the administrator of thehomeless management information system (HMIS) to collect client level information for reporting and analysis on the nature and extent of homelessness, service utilization, and other federal, state and local program performance measurement and management.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  1. Facilitate agency-to-agency collaborations for improvements and innovations in service delivery to the homeless.
  2. Organize and manage the TCHC Planning Council committeestructure to enable the effective identification of gaps in services and assist with the planning and development of solutions and resources.
  3. Plan and facilitate continuing education and training opportunities for service providers and case managers.
  4. Maintain information on resources for service providers on the TCHC website.
  5. Assist with the development of performance management reports and monitoring within the HMIS system.
  1. Oversee data reporting quality of QPR, APR and other HUD reporting requirements from the HMIS system.
  1. Assist with the annual HUDCoC Program grant application process.
  1. Manage the HUD HDX eHIC, PIT and AHAR reporting requirements for the CoC.
  1. Assist member agencies in identifying funding opportunities for collaborative projects and programs.
  1. Other duties as assigned by the Executive Director to support the TCHC mission.

ESSENTIAL SKILLS:

  • A demonstrated desire and commitment to prevent and end homelessness
  • Excellent oral and written communication skills
  • Demonstrated success in public and private grant writing
  • Comprehensive knowledge of homelessness, case management, and homeless service providers
  • Direct experience within a Continuum of Care
  • Experience with HUD programs and grants management
  • Demonstrated knowledge of online tools and applications
  • Demonstrated knowledge of Microsoft Office
  • Ability to work independently with limited direct supervision
  • Experience with reporting and data analysis from an HMIS system

MINIMUM REQURIEMENTS:

Masters degree in Social Services, Public Administration or related field and at least five years direct experience in homelessness, grant writing and management.

Or, Bachelors degree in Social Services or related field and eight or more year’s related experience

SUPERVISORY RESPONSIBILITIES:

This position may require supervisory responsibilities including volunteers and interns.
