Job title COMMUNITY STAFF NURSE CTH - Eastbourne

Responsible to Team leader Principal contacts

Internal: All hospice staff and volunteers, children, young people and families, relatives, carers and friends, visitors to the hospice.

External: Professional and voluntary agencies e.g. Universities, hospitals and hospices.

Environment

The post is based in Eastbourne with regular travel across East Sussex and to CTH

in West Sussex

Job purpose

To provide and ensure a high standard of holistic care to children and young people with a life-shortening condition, and their families, in accordance with statutory regulations and hospice policies within the community setting.

To work as part of the community team's Hard to Reach families project. This includes Networking, identifying local needs, partnership working, providing care and supporting families to access services provided by CTH.

Principal duties and responsibilities

• To provide and be accountable for a high standard of care and support to children/young people and their families.

• To assess, monitor and report changes in child/young person's symptoms and condition, implementing changes in treatment/symptom management according to the hospice practice and protocol, with support from a senior nurse where necessary.

• To act as advocate for the child/young person and their families, supporting their decisions and choices.

• Promote and practice family centred care, ensuring families are involved in planning their child's care needs.

• To work at all times within organisational and professional codes of conduct under the guidance of the Director of Children's Services to be responsible for clinical audit,

service, monitoring and report writing in relation to aspects of community service provided in accordance with the clinical government framework

• To ensure good communication within the team e.g. ensuring dissemination of new information about children/young people and their families.

• To develop specialist clinical skills to deliver high quality palliative care.

• To share the responsibility of professionally developing others within the hospice and externally.

• To carry out duties using safe and appropriate working practice, according to statutory regulations and policies, to ensure the safety of you, other staff, children/young people, and their families.

• Share the responsibility of professionally developing others within the hospice and externally.

• To develop and maintain own professional competence in accordance with NMC Code of Professional Conduct.

Person Specification

Skills & Experience:

• RSCN/RN child branch preferred, however we would welcome RGN/RLDN applications with relevant Paediatric/Palliative care experience.

• Community qualification desirable

• Current registration with NMC.

• Community experience.

• Evidence of personal professional development.

• Mentorship course/Qualification.

• Experience of managing the care of children and young people with complex needs and/or disabilities in community.

• Minimum of 1yr post registration experience.
