Job Summary

  • To have 24 hour management accountability for the provision of specialist palliative care services to meet the individual needs of patients and their families in the in-patient unit and day therapy unit
  • To lead on the delivery, quality and strategic development of nursing services across the hospice, ensuring effective communication across all departments at all levels
  • To be accountable for defining and ensuring that agreed professional standards are reflected in practice within the hospice nursing team
  • To be accountable for the overall management of the hospice nursing team, recruitment, deployment and supervision of nursing staff and volunteers ensuring the co-ordination of all services provided to the patients.
  • Optimise the cost effective use of all resources, providing input into the budget planning process and being responsible for the costs associated to the nursing element of the hospice care.
  • To provide professional leadership, to lead and innovate nursing practice within the speciality throughout the Hospice
  • To promote an environment conducive to the training needs of staff, supporting the Practice Educator role to develop nursing staff and members of the multidisciplinary team, providing support, advice, formal and informal education, reflective practice and clinical supervision
  • To play a key role in the hospice Quality Governance Board, leading on and supporting research and policy development.
  • To deal directly with key external stakeholders, including contribution to any clinical/service related networks associated with the hospice.

Key Responsibilities

Clinical

  • Demonstrate nursing expertise including advanced assessment skills, clinical judgement, decision making and the facilitation of appropriate intervention for patients with specialist and complex palliative care needs in Nurse-Led clinic, in the ward environment and during the referral process where appropriate.
  • Manage own caseload and priorities according to clinical need providing physical care where needed.
  • Act as a resource to the primary health care team providing essential specialist knowledge and advice.
  • Be able to back up clinical decision making with advanced theoretical and specialist knowledge and communicate this effectively to patients, carers and staff across the MDT and with clinical partners
  • In collaboration with the multi-professional/multi-agency teams negotiate care based on needs assessment.
  • To undertake nurse-prescribing practices for patients in accordance with local policies and national standards relating to independent and supplementary nurse prescribing.
  • To facilitate and participate in formal and informal reflective reviews of patient care.
  • Using advanced clinical and interpersonal skills facilitate the team to provide seamless care by managing conflicting viewsand reconciling inter and intra professional differences of opinion and addressing emotional and distressing situations.
  • To have clinical responsibility for the development , implantation and maintenance of agreed clinical policies and procedures and to ensure that review and regular update is undertaken in line with the changing and developing service within MC an nationally
  • Provide specialist clinical support and advice in all clinical areas of the hospice
  • To lead, develop and promote collaborative working within the multi-disciplinary team and external partnerships to provide a high standard of care to patients and their carers
  • To represent the hospice as clinical lead at local, London and National forums (ie LYOL steering groups, PALLE8)

Managerial

  • To manage the in-patient and day therapy services ensuring the effective deployment of staff and volunteers to provide high quality, safe and effective person centred care
  • To manage and support the in-patient nurse manager, clinical nurse specialists and DTU ward sister and support the practice educator
  • Undertake PPRD’s, manage sickness, absence and annual leave levels, dealing with complaints and participating in the disciplinary process where required.
  • To carry out workforce planning for the hospice nursing team to ensure safe staff:patient ratios, team development opportunities and workforce structure fit for purpose
  • To lead on nurse recruitment campaigns, recruit and induct staff and volunteers, ensuringthat probationary reviews are undertaken and mandatory/core training and education is provided and completed.
  • To be the hospice clinical representation in local NHS palliative care strategy groups and network/ systems to ensure awareness and recognition of the contribution of the activities within the Hospice and to identify future development opportunities.
  • To produce reports, statistics and information as requested and analyse and interpret information to review and develop services.
  • To monitor and audit standards to secure and evaluate quality improvements in care.
  • Develop and projectmanage new service interventions/ activities to meet the needs of patients and carers, including formulation of relevant business cases.
  • Lead the service development process for the nursing services at the hospice, to ensure team goals, KPIs and developments are aligned with the strategic plan.
  • Ensure all activities are consistent with Marie Curie Cancer Care and the Hospice’s policies.
  • As a member of the Senior Management Team participate in the development and implementation of the hospice strategy and Annual Business Plan.
  • Contribute to service developments and implement new services to increase the number of patients and carers being supported by the hospice.
  • Manage nurse staffing and ward costs within identified resources and contribute to budget planning for the in-patient unit and day therapy service. (with particular emphasis on the cost effective use of agency resources)
  • To participate in the management ‘on-call’ rota and business continuity plan for the hospice.

Leadership and consultancy

  • Provide leadership in the development of palliative care nursing, at clinical and organisational levels and contribute to the local and national agenda through internal and external networks
  • Role model, innovate and empower staff to make sound clinical decisions and lead the nursing team forward with the patients’ whole journey at the Hospice.
  • To provide specialist clinical support and advice in all clinical areas within the hospice.
  • To work closely with the Hospice Manager, Medical Director and Divisional General Manager to ensure the professional and efficient clinical operation of the hospice.
  • To contribute to the Hospice objectives by leading in Hospice and Charity-wide initiatives to develop nursing practice by planning, implementing and evaluating developments.
  • Collaborate with the Trusts community teams and the MCNS to develop and widen the community reach of the hospice

Audit and Research

  • Lead research based practice in relation to palliative/specialist palliative care, contribute to an evidence based service, through regular review of the literature and application of new evidence to practice as agreed appropriate with members of the multi-disciplinary team
  • Collaborate within the wider multi-professional team to implement nationally agreed standards and audit tools to measure clinical effectiveness.
  • Actively contribute to the palliative care nursing research agenda by engaging in research and in the dissemination of findings through publication in peer reviewed journals and presentations at local / national level
  • Identify and implement service improvements from the findings of research and audit results

Education and Training

  • Within the clinical setting, through active facilitation, contribute to the development of a work based learning culture that actively supports nursing staff to commit to the principle of life-long learning
  • Identify and support the practice educator to respond to education needs and requirements of nursing staff and other hospice staff and volunteers.
  • Provide effective formal and informal teaching and training relating to palliative/specialist palliative care
  • Address the training needs of the wider health community as part of national and local initiatives to enhance palliative care.
  • Liaise with external agencies to promote the hospice services and raise the profile of palliative care.
  • Develop service links with other professionals and agencies to optimize the opportunity for patient groups to access our services.
  • Actively participate in the process of clinical supervision and act as a clinical supervisor / mentor.

Personal & Professional Practice

  • Be aware of professional responsibilities within the NMC Code of Professional Conduct with specific reference to limitations of competence and conduct.
  • Demonstrate understanding of the legal and professional issues surrounding nursing, including specific issues around advanced and specialist practice.
  • Identify own personal and professional learning and development needs in response to the development of the service and clinical lead nurse role
  • Maintain a Professional Personal Portfolio in keeping with PREP requirements.

Clinical Governance

  • Play a key role in Marie Curie Clinical Governance Programme, participating in, and supporting research and audit both within the Hospice, regionally and nationally.
  • To develop key performance indicators that capture service outcomes, ensuring that the hospice quality governance framework effectively monitors organisational performance and activity.
  • Play a key role in the Clinical Governance framework, leading and chairing the Senior Nurse Team working group.
  • Develop clinical protocols and interpret national policies, to facilitate practice developments, benchmarking where possible, to ensure that the Hospice adheres to best practice and makes optimum use of resources to achieve the highest standards of care and service.
  • Maintain a safe environment for patients, visitors, staff and volunteers, ensuring that staff and volunteers are aware of their responsibility in relation to health and safety, cleanliness and infection control.
  • Investigate all accidents, untoward incidents and complaints relating to the ward, reporting to the Hospice Manager and ensure that appropriate action is taken in accordance with agreed policies and procedures.
  • Take an active role in quality assurance initiatives, including the setting and auditing of standards and implementing resulting action plans.
  • Clinical lead involvement in the continuous review and improvement of the hospice governance framework
  • Hospice lead on Peer review of MDT
  • Identify and escalate any new or changing risks/ concerns relating to nursing that need to be incorporated onto the hospice risk register. Ensure that key actions are completed and work with colleagues across the department to effectively communicate and share best practice.

Communication

  • Promote the core MCCC values and hospice ‘one hospice, one team’ culture
  • Develop and maintain robust systems to ensure effective communication between patients, carers, nursing staff, allied health professionals, volunteers, the wider multi-professional team, community and hospital teams and other primary care services.
  • In co-operation with the Hospice Manager and other senior colleagues, develop systems to support user feedback.
  • Ensure effective record keeping and documentation, promoting the introduction and effective use of the hospice’s new computer based patient record system, implementing procedural change as necessary.
  • Promote an open and honest culture which reflects a ‘can do’ and ‘no blame’ attitude
  • To be the hospice clinical lead representative, dealing directly with key external stakeholders and contributing to any clinical/service related networks associated with the hospice. (including NHS London, PALLE8, LYOL strategy groups)
