Job Title: Carers Awareness Trainer (West Leeds)

Status: Temporary – 1 year fixed term contract

Purpose of the Job:

This post is funded for one year to develop, write and deliver short carer awareness training sessions for staff in GP practices, community healthcare staff, Neighbourhood Teams and the social prescribing project (Patient Empowerment Project) within the West Leeds CCG area. Within the context of the Leeds Commitment to Carers and the Carers Leeds Yellow Card scheme, the training will enable GPs, healthcare and non-clinical staff to understand the importance of identifying carers and ensuring they receive the information, advice and support they need through a referral to Carers Leeds. The postholder will work alongside two Dementia Carer Support Workers in West Leeds.

Main Responsibilities:

  1. To develop, write and deliver an innovative carer awareness training session for staff in GP practices and community healthcare settings in West Leeds.
  1. To co-ordinate the training sessions, delivering a minimum of 15 sessions in GP practices, through sending out invitations, booking rooms, organising food and ensuring equipment is in place and functioning.
  1. Raise awareness of the importance of the Leeds Commitment to Carers and the Carers Leeds Yellow Card scheme.
  1. To promote and implement standards of best practice for carer identification and links with Carers Leeds for information, advice and support.
  1. To increase referralsto Carers Leeds services from GP practices and other healthcare providers in West Leeds.
  1. Work with healthcare staff to extend carer support to carers through the development of new clinics or groups in partnership with Carers Leeds (with particular emphasis on support for carers of people with dementia).
  1. Collect feedback and evaluate training with follow up to ensure that the training has been effective.
  1. Keep accurate records of training. Input information into Carers Leeds ‘CharityLog’ database. Fulfil reporting requirements from funding agency.
  1. Visit all Leeds West CCG member practices to promote the Yellow Card scheme, update noticeboards and ensure that staff are fully aware of the importance of identifying and supporting carers.

  1. Ability to develop a carer awareness training course, including materials, suitable for staff in healthcare settings.
  2. Ability to organise and present carer awareness training.
  3. Ability to negotiate with and influence healthcare staff.
  4. Proven ability to give presentations.
  5. Ability to organise own work load and work independently and also to work within a team.
  6. Excellent written and verbal communication skills.
  7. Excellent IT skills and experience
  8. Experience of writing and delivering training
  9. Experience of successfully delivering a project within a fixed time scale.
