SUMMARY

The Care giver is one of the key employees in our Assisted Living & Residential Care Communities. This position has more contact with residents than any other. Staff in this position must provide the facility with a continuous stream of information on resident status, needs and issues. Caregivers must be alert to changes in resident health, attitude and need. They must be able to clearly and concisely transfer this information through the system to ensure proper action is taken. The Caregiver must be able to adapt to the changing needs of the facility and the resident population at any given point in time. They must be able to provide the direct care needed to help residents maintain their highest level of independence. They serve as the lynch pin on the facility care team.

LEGAL REQUIREMENTS

Employees in this position must successfully pass all governmental and corporate required background investigations, such as, but not limited to, state required criminal history clearance investigations.

ESSENTIAL DUTIES AND RESPONSIBILITIES

1.Ability to communicate with and relate well to elder population.

2.Ability to work in confined spaces.

3.Ability to carry trays of food.

4.Provide resident’s guests with meal tickets that are purchased and record information for billing.

5.Knowledge of proper transfer procedures and body mechanics.

6.Proper use of back belts, gate belts, transfer board, and use of mechanical lift equipment.

7.Proper operation of wet/dry vacuum and regular vacuum.

8.Proper operation of toilet plunger and auger.

9.Proper operation of carpet cleaner (carpet cleaning/shampooing machine).

10.General knowledge of oxygen tanks and equipment.

11.Basic knowledge of documentation and record keeping procedures.

12.Ability to read and interpret written procedures and directions.

13.Ability to lift up to 50 pounds weight and carry 100 yards.

14.Ability to lift, bend, stretch, squat, stand and other movements to perform general care giver functions and reach in confined spaces.

15.Manual dexterity adequate for care procedures.

16.Ability to transfer residents with appropriate assistance (i.e. from surface to surface.)

17.Ability to read, comprehend and follow written instructions and company policies.

18.Read the Shift Communication Book, Resident Observation and Evaluation Reports (ROER), and Wellness Center Medication Communication Book immediately after starting work.

19.Observe residents for skin breakdown, lack of appetite, health and behavior changes, foot and toenail problems, and report these and other concerns using a ROER.

20.Immediately alert staff and facility management when a resident wanders inappropriately.

21.Perform special tasks of nursing care when delegated by the RN / Director of Wellness Services.

22.Ability to recognize and report acute changes in a resident’s health status.

23.General knowledge of the aging process and associated chronic diseases.

24.Ability to assist with the modification of resident behavior in accordance with the care plan.

25.Basic computer knowledge.

SUPERVISORY RESPONSIBILITIES

None.

EDUCATION and/or EXPERIENCE

High school diploma or general education degree (GED); or one to three months related experience and/or training; or equivalent combination of education and experience. In addition the individual must have completed the Care giver I program. This individual may or may not be a CNA. The individual must hold a valid drivers license and be insurable to drive the facility van.

LANGUAGE SKILLS

Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence. Ability to speak effectively before small groups of residents, family or staff.

MATHEMATICAL SKILLS

Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio and percent and to draw and interpret bar graphs.

REASONING ABILITY

Ability to apply common sense understanding t carry out instructions furnished in written, oral or diagram form. Ability to deal with problems involving several concrete variables in standardized situations.

DUTIES REQUIRED FOR THIS POSITION

1.Make coffee.

2.Bus tables

3.Operate dishwasher

4.Set tables

5.Serve in dining room

6.Set up and serve in private dining room

7.Take trays to resident's apartments

8.Billing trays to residents.

9.Filling out and collecting meal tickets.

10.General knowledge of special diets (diabetic, etc.)

11.Clean Jacuzzi

12.Recording laundry as done for residents.

13.Doing laundry

14.Answer call lights in a timely manner. (Less than 3 minutes)

15.Keep working environment clean.

16.Assess and respond to emergencies.

17.Assist with A.D.L.'s as listed but not limited to the following:

BATHING: STAND BY, PARTIAL ASSIST, BED BATH, JACUZZI.

ORAL HYGIENE: BRUSHING TEETH, DENTURE CARE, AND MOUTH WASHES.

HAIR CARE: SHAMPOOING, COMBING, SHAVING MALE & FEMALE.

TOILETING: BEDSIDE COMMODE, URINAL.

BOWEL & BLADDER:CHANGING, RECORDING ON B&B, INTAKEOUTPUT POSITIONING OF BRIEFS, ATTENDS, DEPENDS, SKIN CARE.

DRESSING:ASSISTING, TOTAL, PROSTHETIC DEVICES, TEDHOSE.

AMBULATION:ASSIST RESIDENTS AS INDICATED IN SERVICE AGREEMENT.

EMOTIONAL SUPPORT:GOOD LISTENING SKILL, NON JUDGMENTAL

PERICARE:CLEANING

HEARING AID:PUTTING IN, REPLACING OF BATTERIES, CLEANING

NAIL CARE:CLIPPING, FILING, and TOES & FINGERS DIABETICS NEED TO BE TASK DELEGATED.

18.Perform duties as assigned per the care giver duty roster.

19.Be aware of individual resident service plans and the services to be provided on a scheduled and oncall basis.

20.Report changes in resident conditions via the staff notes, communication book and incident reports.

21.Fill in ADL task sheets before leaving the shift.

22.Answer the phone in a timely and professional manner and record messages accurately when appropriate.

23.Maintain confidentiality of staff, residents and company.

24.Attend staff meetings and in service meetings. All staff meetings are mandatory unless specifically designated by the Executive Director or Assistant Administrator

25.Take vital signs: BP, TEMP, PULSE, and RESPIRATION.

26.Maintenance and use of medical equipment

27.Knowledge of measures & weights: cc's, ounces, pounds, etc.

28.Report verbally and in writing, on individual resident’s abilities and potential changes to the service plan. This should be done through notations in the progress notes and shift notes and through verbal discussions with supervisors and care plan team members.

29.Assist other departments as needed to ensure a safe working environment is maintained. This would include reporting safety hazards promptly, changing light bulbs in units and easily accessible areas as needed, logging all maintenance issues and repair needs.

30.Other duties as required from time to time to ensure good continuity of service.
