JOB DESCRIPTION FOR TENANCY SUPPORT WORKER

3. ACCOUNTABILITIES

3.1 Overall Purpose

To provide support to adults with mental health needs who have moved on from Hazelmead House, and enable them to sustain independent living.

3.2 General Operations

To work within the policies and procedures of Jephson Housing

Association and Hazelmead House.

3.3 Liason with External Agencies

To work with a wide range of support providers to promote the welfare of

residents.

3.4 Health and Safety

To contribute to safe working at the Project.

4. PRACTISE GUIDANCE

4.1 Overall Purpose

To provide support to adults with mental health needs who have moved on from Hazelmead House, and enable them to sustain independent living.

4.1.1 To assess the support needs of identified residents and develop a Support Plan.

4.1.2 To support the resident to successfully adhere to the Support Plan.

4.1.3 To monitor the residents achievement against the Support Plan, and to review support offered as appropriate.

4.1.4 To ensure residents are aware of welfare benefits available, and provide support to claim these.

4.1.5 To ensure tenants comply with the responsibilities outlined in their occupancy agreement.

4.1.6 To monitor the welfare of each resident and promptly raise concerns with line manager and/or external support agencies.

4.1.7 To support residents to access community services and specialist facilities.

4.1.8 To record case notes for all tenants and maintain these in accordance

with established procedures.

4.2 General Operations

To work at all times within the policies and procedures of Jephson

Housing Association and Hazelmead House.

4.2.1 To maintain records and provide reports to enable evaluation of the outreach service.

4.2.2 To attend supervisions, meetings and training.

4.2.3 To keep up to date with changes in legislation and practice relating to the client group.

4.2.4 To work flexibly in accordance with the needs of the Project.

4.2.5 To undertake other duties which may be required by the line manager.

4.2.6 To work at all times within the Association’s Race Equality and

Equal Opportunities policies.

4.3 Liaison with External Agencies

To work with a wide range of support providers to promote the welfare

of residents.

4.3.1 To work with a range of local voluntary and statutory agencies

to progress mental health issues.

4.3.2 To act as an advocate for tenants.

4.3.3 To work closely with mental health professionals and others offering support to residents in order to progress their welfare.

4. 4 Health and Safety

To contribute to safe working at the Project.

4.4.1 To be aware of and adhere to established Health and Safety

procedures to ensure personal safety and that of residents and visitors.

4.4.2 To make Heath and Safety checks as required.

4.4.3 To identify Health and Safety risks and action as appropriate.

4.4.4 To contribute to the risk assessment of new referrals and existing

residents.
