This application form is to be used by new applicants, current grantees with new projects, or former grantees with a new project. Please complete all questions below, paginate the application form and proposal narrative, and adhering to page limits. Please attach a proposal narrative (maximum of 4 pages in 12 point font) incorporating the following points:

1.Organizational Overview: In one or two paragraphs, please:

  • Summarize your organization’s history.
  • State the organization’s mission, goals and population served.
  • Outline current programs, activities, and organizational structure.
  • Highlight organizational accomplishments.

2. Problem Statement: In one paragraph, please state what needs or issues the project addresses.

3.Project Description: Describe the project for which support is sought. How does this project address the root causes identified in the problem statement? How will the overall status of Jewish women and/or girls be improved by this project? Please be specific regarding the demographics of the target population including gender, religion, age, sexual orientation, race and people with disabilities. Please indicate if this is a new or ongoing effort. If applicable, please describe how participants will be recruited and whether your organization will collaborate with any other organizations or individuals on this project.

4.Goals and Objectives*: Please answer the following questions pertaining to the identified shift(s) of Social Change your project is seeking to address. If your project addresses more than one of the indicators, please select the top two that most clearly reflect the work outlined in this application. Be sure to clearly outline the overall goals and specific, measurable objectives for the project. Please make sure your objectives are reasonable and achievable within the funding period, and that they map out a clear strategy for achieving the shift(s) of social change you identify above. Please state any additional goals and objectives of the project that might not fall under one of these shifts of social change.

  1. Shift in Definition and Reframing of Issues: The issue is defined differently in the community or larger society as a result of your work
  2. Existing Definition: What is the issue you would like to redefine/reframe?
  3. Goals: What do you want the definition to be as a result of your work?
  4. Strategies: What strategies will you use to make the shift happen?
  5. Evaluation: How will you measure success?
  6. Shift in Individual and Community Behavior: People are behaving differently in the community or larger society as a result of your work
  7. Existing Behavior: What is the behavior you want to change?
  8. Goals: What do you want the behavior to be as a result of your work?
  9. Strategies: What strategies will you use to make the shift happen?
  10. Evaluation: How will you measure success?
  11. Shift in Critical Mass and Engagement: People in the community or larger society are more engaged as a result of your work
  12. Existing Engagement: What is the situation you want to change?
  13. Goals: What do you want the outcome of your work to be?
  14. Strategies: What strategies will you use to make the shift happen?
  15. Evaluation: How will you measure success?
  16. Shift in Institutions, Systems, and Policies: An institutional, organizational, or legislative policy or practice has changed as a result of your work
  17. Existing Policy/Practice: What is the institutional response, policy or practice you want to change?
  18. Goals: What do you want the outcome of your work to be?
  19. Strategies: What strategies will you use to make the shift happen?
  20. Evaluation: How will you measure success?
  21. Maintaining Past Gains: As a result of your work, past gains have been maintained, generally in the face of opposition
  22. Current Policy/Practice/Definition/Situation: What past gains do you want to maintain?
  23. Goals: What do want the outcome of your work to be?
  24. Strategies: What strategies will you use to make the shift happen?
  25. Evaluation: How will you measure success?

5.Evaluation: How will you know you have achieved the social change shift(s) your organization desires? Who will be involved in evaluating the project -- staff, board, constituents, community, consultants? How will the evaluation results be used?

6.Funding Status and Plans: What is the current source of funding for this project? How will you secure additional funding? How will the project be maintained after support from JWF ceases? Please be as detailed as possible.
