J-1 Scholar DS 2019 Appointment Process Checklist

Please read carefully the checklist below and submit all necessary documents with the appropriate DS 2019 Data Sheet: Form A, to be completed by the hosting UMass Dartmouth department, Form B to be completed by the scholar, and Form C for dependents, if applicable. The International Student & Scholar Center cannot prepare the visa paperwork without these documents.

ð  1. Form A, for department, completed and signed by the hosting department, including forms for

o  export control

o  intellectual property

o  language proficiency

o  host department agreement

ð  2. Form B, for exchange visitor, completed and signed by prospective exchange visitor.

ð  3. Form C, for dependents, if applicable; completed and signed by prospective exchange visitor and dependent(s).

ð  4. Exchange visitor's current resume or curriculum vitae.

ð  5. From Exchange visitor's passport: copy of data page(s) including passport expiration date, plus any U.S. visa stamps.

ð  6. If currently in the U.S. or if visitor left the U.S. less than one year prior to the proposed start date of this appointment, provide the following. (If you are/were in visitor status (B-1/B2, WB, WT), copies of your passport, visa and I-94 record will suffice.)

ð  Copy of all Forms DS-2019 if in J-1 status.

ð  Copy of all H-1B Approval Notices, if in H-1B status.

ð  Copy of all Forms I-20, if in F-1 status.

ð  Copy of I-94 Admission Record.

ð  Copy of all Employment Authorization cards, if any.

ð  Copy of your three most recent pay stubs if you are currently in the U.S. in the U.S. on H-1B, O-1, or TN status.

ð  Copy of approval or waiver of J-1/J-2 two-year home residence requirement (212e), if applicable, or copy of application receipt if you have applied for a waiver but it has not yet been approved.

ð  7. Financial documents as described under “Financial Support” on page 2 of Form B. All documents must be original, written in English and converted to U.S. currency.

Official translations: Documents that are not in English must be translated by someone other than the applicant or applicant ‘s immediate family who is competent in both English and the language of the documents. The translation must be signed and certified with the following statement: “I, (name), hereby certify that I am competent to translate from the (name language) language to English and that the attached is a true and accurate translation of the original document.”
