SECTION 4 - TERMS AND CONDITIONS

1. All forms must be received by Monday 3rd June 2013. Bookings will be accepted and confirmed only when full payment has been received.

2.Early bird rate:Bookings and payment must be received by Monday 29th April 2013.

3. Cancellation policy: A full refund will be given on all cancellations received on or before 17 May 2013. An administration charge of £15 will be made for all such cancellations. Any cancellation received after this date will not receive a refund. Cancellation requests must be made via email to . Verbal cancellations will not be accepted.

4. Substitution policy: The delegate can change the name on the booking up to two weeks before the Conference by contacting . Verbal substitutions will not be accepted.

5. This Booking form constitutes a legally binding contract. It may be necessary for reasons beyond the control of The Institute of Work, Health and Organisations, The University of Nottingham to change the content and timing of the programme. In the unlikely event of the programme being cancelled, The University of Nottingham will automatically make a full refund but disclaim any further liability.

This conference will bring together those involved in clinical psychology training to share ideas about achieving meaningful service user and carer involvement in training.

Four parallel sessions will run. Topics include overviews of involvement within courses and examples of involvement in teaching, placements and research.

Colleagues from other disciplines with an interest in involvement in mental health training are also welcome to register.

A conference programme will be available shortly.
