RFP 13000, Translation Services

The following are changes, revisions and clarification to RFP #13000:

  1. Page 10, Paragraph 2 has been divided into two separate paragraphs as follows:

2a. BUSINESS EXPERIENCE

Describe your experience providing interpreter services during the last two calendar years. List and provide evidence such as examples of your experience with private business organizations as well as individuals. Failure to provide this information may be grounds for finding your proposal nonresponsive.

2b. REFERENCES

Proposer to include five (5) references with whom you currently do business that can confirm your experience. (See page 39 of RFP.) A minimum of one reference must be a non-DSHS referent. Proposer to include the name and direct telephone number of the person at each company who can verify your agency has provided interpreter services for that company. Failure to provide this information may be grounds for finding your proposal nonresponsive.

B.Page 10, Paragraph 4 First sentence has been changed. Paragraph to read as follows:

4. CRIMINAL HISTORY BACKGROUND CHECK

At time of site visit, proposer shall have completed and have on hand for review Washington State Patrol criminal history background checks (within the last two years) on all employees, volunteers or subcontractors who will be providing services under this contract. Any changes in criminal history after contract award shall be reported immediately to the Department of General Administration, Office of State Procurement. All criminal history background checks must be kept on file with the agency.

Clarification: Instant Internet printout from the Washington State Patrol website is acceptable. Their website address is Agency requesting that WSP mail copies to them need to annotate that requested copies are for contract compliance.

C.Page 11, Paragraph 5, Clarification

Exhibit F is a new form that must be completed and signed by all interpreters by time of site visit.

Page 11, Paragraph 5, (h) Delete the letter (h). This is a stand alone paragraph further describing the requirements for DSHS SELF-DISCLOSURE FORM

D.Page 12, Paragraph 8, Change first sentence to read:

After award of contract, Contractor will provide evidence of or proof of a comprehensive drug-free workplace program.

E.Page 14, Paragraph 18, Delete requirement in its entirety.

F.Page 15, Paragraph 24, Clarification

Exhibit D, pages 1 and 2 – this form and instructions will be revised and reissued at a later date. Sections related to mileage on the current form do not need to be completed if the interpreter agency is not claiming mileage.

G.Page 17, Paragraph 1, bullet number 4, and page 18, paragraph 8, third sentence regarding reference to scheduling database - Clarification

Proposers must have the ability to electronically schedule, track and report services provided. The department does not specify whether individual software components or an integrated system is required.

H.Page 18, Paragraph 8, Clarification

The contract addresses only those services provided under this contract and not other services provided by the agency. GA/DSHS cannot provide advice outside the scope of this contract.

I.Page 19, Paragraph 13, Revise first sentence to read:

Contractor shall provide an original invoice and the original signed copy of the “Appointment Scheduling and Confirmation Record”.

J.Page 19, Paragraph 13, Under paragraph for “Other authorized contract users” delete last sentence.

K.Page 20, Paragraph 14.B.e. Correction to first sentence:

When appointments do not meet the criteria given in “d” above, Contractor may bill MAA a minimum of four units per encounter.

L.Page 22, Paragraph 16, Change first sentence to read:

Contracted agencies, their employees and/or subcontractors are prohibited from applying undue influence on DSHS clients, employees and service providers by:

M.Page 23, Paragraph 18, Clarification

Proposers are to submit an original proposal and ten copies of their entire original proposal for a total of 11 submittals.

N.Page 23, Paragraph 19, Change second to the last sentence to read:

An affidavit of completion of the checklist must be signed and dated (in ink) by the interpreter after completion of the orientation.

  1. Page 25, Paragraph V.1.6. Delete Financial Statement in its entirety.
  1. Page 25, Paragraph V.1.7. Delete Drug-free policy/statement document. (This requirement is to be submitted after award of contract.)

Q.Page 26, Paragraph VI, Change second sentence to read:

Letters of intent or business agreements for qualified, authorized or certified interpreters, to verify these language needs are met, must be an original signed copy; and must have been signed within the two years prior to submission of a bid on this contract.

R.Page 26, Region 3, Delete Cambodian from the language requirements for this region.

S.Page 27, Region 4, Delete Samoan from the language requirements for this region.

T.Page 27, Region 5, Delete Tagalog from the language requirements for this region.

U.Page 28, Example table has been revised and is attached.

V.Page 29, last sentence on the page has been corrected to read:

The maximum points your firm may receive for questions A through G are 100 points.

W.Page 40, Offer and Award, under Contract Award, last sentence has been corrected to read:

This is a Partial award for INTERPRETER SERVICES, SPOKEN LANGUAGE.

X.Page 41, CHECK LIST has been revised and is attached.
