CONTRACT FOR NURSE FAMILY PARTNERSHIP SUPERVISORY SERVICES
Amendment Number 1
THIS CONTRACTamendment is entered into in duplicate originalsand is effective August 31, 2017 between THURSTON COUNTY, hereinafter “THURSTON,” and LEWIS COUNTY, hereinafter “LEWIS.”
THURSTON and LEWIS mutually agree to extend the existing Contract for Nurse Family Partnership (NFP) Supervisory Services for four (4) months. In accordance with Section 2. Term, the parties agree that the Contract shall be amended as follows:
- The Contract term will be extended for an additional four (4) months through December 31, 2017.
- For the term of September 1, 2017 through December 31, 2017, Section 4. Compensation, shall be revised to update the time period as follows:
For NFP Supervisor services set out in Section 3 for the time period of September 1, 2017 through December 31, 2017, LEWIS will reimburse THURSTON at an hourly rate of $69.11 for NFP services, which includes FICA, Unemployment, Disability, L&I, Retirement, and Medical Benefits. Additionally, mileage will be paid at $.535/mile. Compensation for NFP Supervisory services provided by THURSTON above 4 hours per week will be paid by LEWIS at the NFP Supervisor current hourly rate of $69.11/hour.
- Except as expressly provided by this Contract Amendment Number 1, all other terms and conditions of the original 2017 Contract for NFP Supervisory Services remain in full force and effect.

IN WITNESS WHEREOF,the parties hereto have caused this Amendment Number 1 to be executed, and the parties hereby ratify and confirm any work performed on or after August 31, 2017 but prior to the date of formal execution by the parties of this Amendment Number 1 that falls within the scope of this Contract and is consistent with its terms.
