IYSS After Care Provision“Staying Put” Excluded Licence Agreement

  • This agreement sets out the terms and conditions whilst you are stayingwith: (Insert Landlords name IE: the foster carer or supported lodging provider) at (insert address).
  • This agreement is between ………………………… (The young person) ...... (The landlord) and Torbay Children’s Service IYSS (After Care Provision) Team in respect of the above address.
  • The date of commencement of your licence in “Staying Put provision at the above address is: ………………………. and the Length of the license is 6 months and will be reviewed half yearly.

Costs Rent:

Room and rent84.00

Supported Lodgings “Staying Put” Placement licence support costs per week is broken down as follows:

Direct support 100.00

Food/full board 32.00

Heat and light to common area’s 15.00

Water rates 5.50

Cleaning common areas 7.00

Use of furniture 5.00

Use of household equipment 8.00

Use of laundry facilities 7.50

Use of TV/video/radio 10.00

Use of telephone/internet 10.00

Total 200.00

The young person will contribute £20 from their allowances/wagesfor food.20.00

The payment shown will includeSupport and guidance in:-

Budgeting

Debt counselling

Cooking and cleaning training

Support in healthcare

Social skills training

Network training

Personal safety training

Sexual health training

Other support as necessary

Obligations:

Torbay Children service agree to:

Support the (Insert YP name) to claim Housing benefit.

Maintain a pathway plan in partnership with (Insert name) and shared as agreed by the Young Person.

Complete visits and keep in touch with (Insert name) and to (insert landlords name) in line with National Standards and to attend placement meetings.

Provide a letter to Housing benefit to explain that (Insert YP name) is a care leaver and their legal status, is vulnerable and to request that Housing benefit goes to the Landlord direct.

Provide financial support in line with published allowances in addition to Housing Benefit or equivalent entitlement and any contribution from the Young Person if appropriate to meet the agreed rent and support costs between parties.

The Landlord (Insert name) and agency (Insert Company name) agree to

Provide suitable accommodation and a bedroom for sole use of the tenant.

To maintain the supported lodgings in line with fostering regulations for any other placed children and standards for supported lodgings.

The agency to support the carer.

To offer support and guidance to the young person as outlined in the support schedule.

To complete any DBS checks on householders and adults residing in the property.

To advise the Local Authority of any change of circumstances or significant event.

(Insert YP name) agrees to:

Pay £20 per week for food from her entitlements or/and wages to the landlord.

To make a contribution towards the rent. Thiswill bedependent upon whether a young person is in full time/part timeemployment, education or training. The amount is to be agreed at Placement Planning meetings if a decision is made by Housing benefit that there claim is reduced due to their income.

To meet and complete pathway plans with the allocated Personal Advisor and draw up an Individual support Package in consultation with the youngperson detailing roles and responsibilities of all people involved with theyoung person while in Staying Put provision.

To be respectful of neighbours and the local community including;not to play any radio, stereo, television etc so loudly that itcauses a nuisance or annoyance to neighbours inside or outside the lodgings.

Not to participate or encourage others to hold social gatheringsbetween 2200 hours to 0800 hours.

Not to invite others to stay overnight without discussion with the landlord.

Not to commit or allow invited visitors to commit any form ofharassment to others or illegal activities.

If there are any damages to the property or equipment this is to be reported to Children services (IYSS After Care Provision), the agency and/or the Out Of Hours Service at weekends, promptly. Anywilful damage will result in a contribution to the repairs orreplacement by the Young Person (Insert name).

Pets are/ are not allowed.(delete as appropriate)

Smoking or the burning of incense/candles is not allowed in the Staying Put Lodgings.

Smoking is allowed in (insert specified area).

To ensure that all rubbish is placed in the communal rubbish binprovided.

To attend the review/evaluation meeting.

On moving out the young person is to remove all personal belongings,leave the room clean and tidy and return the key/s to the Landlord.

The replacement cost for any lost key/s will be the responsibility of theyoung person.

Ending the stay in “Staying Put”

The young person shall give 24 hours notice to the Landlord if they wish to leave the “Staying Put” Supported Lodgings before the planned end of the licence date.

The young person can be asked to leave immediately if the agreement is broken or they wilfully refuse toparticipate with the aims of their stay.
