Notes to Instructor: This form is used for you to provide your relevant Primary Branch the necessary information to comply with The Pony Club rules and other legal requirements. While the Branch may have some information about you stored on the Instructors Database there will be some areas that are missing, so the intention with this document is for you to provide that missing information. The fields found on here are the same fields required by the Instructors Database.
