Instructions for Parent Surveys
Review the survey questions (below) and print the Images from Parachute (if you are using them), as you prepare your session. This will assist you to make sure that you cover the key topics in the session.
Print two copies of the following pages for each caregiver you are expecting, one before session and one for after the session. Be sure to mark one as pre session and one as post session. Provide extra pencils/pens for them to use and give them a few minutes to fill out the survey before the group starts. For the first survey, tell them just to answer the questions in the boxes, not the last two questions.
Explain that it is OK if they don't know the answer because they will get another chance to do the survey after the program is finished. Be available to assist any caregiver who is struggling with understanding the questions.
After the session is completed, ask caregivers to fill out the same survey again. Explain that these answers to the surveys will help you provide better programs and information for caregivers. You may add questions to the survey or combine surveys if you are discussing multiple injury topics.
Thank the caregivers for their assistance in completing the surveys.
To compare the pre and post responses for a person, add the number of correct responses in the pre survey and then in the post survey and compare the change in number of correct responses. If you want to create a percentage correct for the pre and then the post, divide the total number of correct answers by the total number of questions and multiply by 100 = % correct. Comparing an individual’s pre and post surveys may also help you to identify injury messages that are still unclear to a caregiver.
It is a good idea to keep a spreadsheet or table that lists each of the sessions delivered, number of caregivers attending, topic presented and results comparing pre and post correct answers.
