INSTRUCTIONS FOR COMPLETING THE FALL SURVEY ENROLLMENT REPORT

Fall Survey Data Collection

Once you have clicked on FS and are in the Welcome Page, you must click ENTER to gain access to the login screen. You will need to contact your web user administrator for your user name and password.

MAIN MENU: Your county and district will show on this page.

  1. Coordinator (ModifyContact Information). Click on the Coordinator Contact button to review the information correct the contact information for your school, if necessary.
  1. SelectSchool (ClickSELECTSCHOOL for a list of your schools.) Choose a school to begin entering data and a new screen will come up asking what program you would like to go into. Please note that totals for rows and columns are calculated (summed across the row or column) To correct a total, you must correct the inaccurate data in the row or column. In order to save the informationyou must click theUPDATEbox. (For descriptions of the data elements, refer to the section entitled Description of Data Elements for the 2009-10 Fall Survey Report).

Please note: because many classified students do not spend 100 percent of the day in either self-contained or regular classroom settings, the percentages of time are included in the headings to assist you in making a determination based on where the student spends the majority of class time. The second category corresponds to what used to be covered by the term mainstreamed and is reported by grade level. The third category corresponds to self-contained programs and is reported by grade level. The fourth category includes the subgroup of students reported by program type.

  1. Print a report to check your data for accuracy and provide a record.
  1. Finalize (Finalize your data).

When finalizing your data you will receive either an error page or a log-out page. If you receive the error page, you have errors in your data and you must correct them. Unless you correct the data, you will not be able to transmit. If you had to correct data, you must finalize the data.

When all data is correct you have the option to print a District Report and /or School Reports. The District Report will have a total by Grade, Race and Sex for all your schools combined. The SchoolReport will have to be printed for each school and this would have all your school data that was inputted.

g. Exit (this will take you back to the Homeroom Page).

This information will be reported on the 2009-10 School Report Card.

When all information has been finalized for the district, you must also print the TRANSMITTAL FORM (accessible from the WELCOMEpage). Complete this form, including certification by the Chief School Administrator and the Special Education Coordinator. Return the form to NJ DOE by faxing it to FSV at (609) 984-6756.
