Proposal Guidelines:
Please attach additional pages as needed and provide documentation for each of the categories listed.
· Statement of Need – Describe the area of student achievement to be addressed and provide any data that supports the need. Please include how this grant addresses district and/or campus goals.
· Objectives – State measurable objectives in terms of student behavior or performance.
· Description of Proposed Project/Activity – Describe what the funds will allow the program to achieve and how it will be innovative. List activities and timelines.
· Sustainability – If funded, how will the program/project be continued in the future? What will be the recurring costs?
· Evaluation – Create and attach a measurement tool that will track program outcomes.
Budget Considerations:
Please be as specific as possible when determining budget needs. Budget items might include things such as supplies, equipment, or other materials necessary to complete project.
