CITATION INFORMATION TO DRIVER

Sir/Ma’am here is your driver’s license and registration.

Here is your citation…

1. Point System:

If you look at the upper right hand corner, you will see that the fine is $______and just to the right you will see that this is a ______point violation. Do you understand Wisconsin’s point system?

Yes (go on to the next step)

No

If you accumulate 12 points in a 12-month period of time, your license will be suspended.

2. Mandatory Appearance and Court Info

In the upper left hand corner, you will see that this is not a mandatory court appearance. That means that you don’t have to appear.

3. Disputing the Citation

If you would like to dispute the citation, you can show up at the court time and location indicated on the citation or you can mail a not guilty plea along with a copy of the citation to the address on the citation. There are instructions on the reverse side of the citation explaining this further.

4. Paying the Citation

If you would like to pay the citation, you can mail the dollar amount to the address listed along with a copy of the citation to the address listed on the citation or you may pay the citation in person at the clerk of courts office or at the police department. The payment must be received prior to the court date.

5. Consequences for Ignoring

If you ignore the citation and do nothing, a warrant for your arrest could be issued or your license may be suspended.
