  • Ohio 4-H Scholarships were established in 1976 to help young people continue their efforts toward becoming self-directing, productive, and contributing members of society.
  • There are currently 36 statewide 4-H scholarships being offered.
  • You will need to submit an application online to be eligible for state 4-H Scholarships! (New in 2014!)

  • In addition to personal information, before beginning the online application for the State 4-H Scholarships, please make sure you have the following information available (be ready to copy and paste from your computer).
  • High school Grade Point Average (GPA) and rank
  • Academic honors you have received
  • If you are or will be an Ohio State student, your OSU username.# and ID# can be found on your receipt of admission application email
  • Information about your current or anticipated college or university (name, city, state)
  • Intended academic major
  • Detailed information about your major 4-H leadership roles; you should know the names of the roles, the years, level(s), duties and accomplishments
  • Detailed information about major 4-H honors that you have received
  • Information about your educational, career, or personal goals and what impact 4-H has had on those ambitions
  • Personal statement explaining why a 4-H scholarship would be important to you achieving a college education AND how 4-H has contributed to your personal development, leadership and other skills and to your likely success in life

2017 FAIRFIELD COUNTY 4-H SCHOLARSHIP APPLICATION

Individuals Applying for Fairfield County 4-H Scholarships (due January 6, 2017 at 4:00 p.m.)

  • Must be at least a high school ‘senior’ or college ‘freshman’ (seniors preferred).
  • Can attend ANY post-secondary college/university to be eligible for County 4-H Scholarships.
  • Must complete this application (including reference letter and copy of transcripts), ALONG WITH the Ohio 4-H Achievement Form and turn in everything (as a hard copy) to the Fairfield County Extension Office by January 6, 2017 at 4:00 p.m.
  • Winners must be current 4-H members in Fairfield County in the year in which they are awarded a county 4-H scholarship.
  • Winners will need to provide acceptance letter to selected school, upon receiving scholarship.
  • Criteria used in selections of Fairfield County 4-H Scholarship winners will include: Major 4-H Activities, 4-H Project Achievement, Personal 4-H Story, Extracurricular Activities, Work Experience, Potential for Success, and Personal Statement.
  • To apply for scholarships, members must complete and submit as one application submission (please staple all items in this order in the upper left hand corner):

  • A a current transcript of your grades (for reference purposes only),
  • A one-page letter of support from your academic advisor, guidance counselor, teacher, community leader, or employer.
  • Fairfield County 4-H Scholarships (including all items as one application submission) are due to the Extension Office by 4:00 p.m. on Friday, January 6, 2017.

Directions: When applying for the Fairfield County 4-H Scholarship you should submit this Fairfield County 4-H Scholarship Application Form and attach the following items for reference purposes: 1) a current transcript of your grades; 2) a one-page letter of support from your academic advisor, guidance counselor, teacher, community leader, or employer; and 3) Ohio 4-H Achievement Form (which includes sections A-G, photo page, and a two-page, single-sided, double-spaced personal 4-H story that shares how 4-H contributed to your personal development, leadership skills, and/or career plans).
