In accordance with Section 3115.42(B) of the Revised Code and Local Rule 39, you are hereby advised that a Petition for Registration of a Foreign Support Order from the State of ______has been filed with this Court and that you have an opportunity to contest the validity or enforcement of the order. Attached hereto is the Request which includes a certified copy of the Order being registered.

You are hereby advised of the following:

(1) That a registered order that is confirmed pursuant to section 3115.43 or 3115.44 of the Revised Code is enforceable as of the date of registration in the same manner as an order issued by a tribunal of this state;

(2) That a hearing to contest the validity or enforcement of the registered order must be requested pursuant to Section 3115.43 of the Revised Code no later than twenty (20) days after the date of mailing or personal service of the notice;

(3) That failure to contest the validity or enforcement of the registered order in timely manner will result in confirmation of the order and enforcement of the order and the alleged arrearage and precludes further contest of that order with respect to any matter that could have been asserted;

(4) The amount of arrearage under the support order is ______.

Please be advised that upon the registration of an income withholding order for enforcement, this Court shall issue a withholding notice to your employer pursuant to sections 3113.21 to 3113.219 of the Revised Code.

Should you wish to contest the validity or enforcement of this registered order, you must request a hearing no later than twenty (20) days after the date of mailing or personal service of this Notice of Registration by filing a motion with this Court.

You may also seek to vacate the registration, to assert any defense to an allegation of noncompliance with the registered order, or to contest the remedies being sought or the amount of any arrearages pursuant to section 3115.44 of the Revised Code.

Failure to act in a timely fashion as set forth above shall result in the order being confirmed by operation of law.
