I. RECOMMENDATIONS
At this hearing the Judge may order the children returned to parent, or order the implementation of a concurrent plan. Here are a few suggestions. You may use those that apply to your case.
· That the child(ren) be returned to the parent(s)
· That the child(ren) remain a ward(s) of the court in the care and custody of DHS/CW with continued placement in foster care, relative foster care or parents (whichever is appropriate to your case)
· That the concurrent plan of adoption be implemented (or other identified plan that you are recommending. Other concurrent plans are: guardianship, permanent foster care, or another permanent planned living arrangement)
· That an extension of a specific period of time (60 days, 90 days, 180 days) be granted in order for the parents to continue services. (This usually occurs when the parents are close to being ready for reunification.)
II. STATUS OF CHILD(REN)
Include one paragraph of general information, then one paragraph specific to each child. Provide information that will help the court to know the child such as descriptions of what they look like, what they like/dislike and what activities they are involved in. Address placement of child and how child is doing in current placement, services needed, parental and sibling visitation-when it occurs, how the child reacts to parent. Discuss Emotional/Behavioral issues; Medical/Dental issues, i.e. are immunizations up to date, what medications is the child taking; and Education, i.e. what grade is the child in, how school is going.

(Are parents in compliance with the Action Agreement? It will probably be necessary to address each parent separately. What is the visit schedule? How are visits going?)
IV. CONCERNS
Express any concerns that you might have about the child, the parents, or current situation.
