IDAS form 12—Taking or interfering with artesian or sub-artesian water

This form must be used for development applications for operational work that involve taking or interfering with artesian or sub-artesian water.
You MUST complete ALL questions that are stated to be a mandatory requirement unless otherwise identified on this form.
