I. DESCRIPTION OF PROBLEM AND PROJECT (2 pages)

  • Project description

Briefly explain the problem that the project is designed to remedy, emphasizing the quality issues to be addressed. Describe the scope of the problem and its consequences if not addressed. Provide an overview of the project and its rationale, the need for improvements in this particular area, the efforts that have been undertaken so far and the value-added that the project is expected to bring.

  • Related work

Briefly describe earlier activities and projects in this area, showing any connections with each other and this project. These activities and projects may be undertaken by the applicant, others at GW, or published in the literature. Briefly describe any relevant current activities being undertaken by the applicant or administrators in other programs, departments, or schools; explain the linkages among these projects.

  • Overall goal(s)

Describe the quality improvement impact of the project. Focus on the educational, administrative, economic or customer-service contributions, and describe why the project is important for Health Sciences.

  • Objectives of the project

Describe the objectives and expected outcomes, or direct effects, of the project. These should be measureable, verifiable and time-bound

II. PROJECT PLAN (6 pages)

  • Activities

Describe the activities that will be put in place to achieve the goals and objectives. Provide a detailed description of intended procedures. Discuss potential barriers to project implementation and alternative approaches

  • Project Management

Describe what will be done and by whom, including communication among team members and supervisors

  • Milestones and Deliverables

Describe the deliverables (tangible or intangible products or processes) produced as a result of the project, and the milestones established to measure progress toward those deliverables.

  • Evaluation

Describe the evaluation methods that will qualify and quantify the effect of the project on quality, including process (formative) measures and outcome measures.

  • Sustainability

After the funding period ends, how will the effects of the project be sustained over time? Describe how the results of the project will be disseminated.

  • Community of Practice

Describe how the project can contribute to or create a Community of Practice in HS.
