PARENT DECLARATION

□ I consent for my child/children to participate in any incursions conducted as part of the Vacation Care Programme

□ I consent to my child watching G rated movies.

□ I give permission for my child/children to be photographed while at the Centre and for the photos to be used for promotional purposes.

□ I am aware that if my child is sick he or she may not attend the programme, or a phone call may be made during the programme requesting my child to be collected.

□ I hereby agree that St Andrew’s School Inc and all staff, whether paid or voluntary, shall be exempt from all liability including any loss, injury or damage sustained by my child while participating in the programme.

□ I further give consent for medical or dental assistance to be given to my child/children without notice to myself in the event of an emergency should this be deemed necessary and I cannot be reached by telephone.

□ I hereby give permission for the appropriate St Andrew’s School staff to use their judgement in obtaining the best such service as required. I understand that any cost incurred will be my responsibility.

□ I understand that St Andrew’s School operates as a nut restricted environment and that nuts and food containing nuts are not to be sent to School or Vacation Care.

AIM

The School aims to provide quality care for children from ELC to Year 2. The service intends to create a safe, caring and stimulating recreational environment which caters for individual needs. Activities are diverse and are aimed to engage all children.

FEES AND CHARGES

Whole Day (8.00am to 6.00pm or part thereof):$95 per child per day

A late fee of $25 will be charged after 6 pm for each 10 minutes or part thereof

Bookings and inquiries –

Bookings close on Friday 8th December, 4pm. Numbers are capped each day so early bookings will ensure you receive your preferences.

PLEASE NOTE

Please advise by telephone if your child is not attending on a particular day for which they have been booked.

• Care is available only between the hours of 8.00am to 6.00pm. Please be prompt with pick-up times.

• Vacation Care caters for a limited number of children with ELC students given priority as per licensing requirements.

• Staff/student ratios are dependent on number with all staff having appropriate qualifications.

• Parents/Carers must sign children in each day upon arrival and out each afternoon.

• Please ensure all medication is named. All medication must be handed to staff with a Medication Plan and/or Action Plan.

• Children should bring a named water bottle, recess, afternoon snack and lunch unless the programme states otherwise.

• St Andrew’s ELC is a nut-restricted environment to protect children with allergies. Parents are reminded not to send any food items containing nuts.

• The ELC is also an egg-restricted environment. However, eggs are allowed if they are used as an ingredient in a cooked cake, biscuit, etc.

• Please do not send noodles for lunch or food requiring heating up.

• Some activities may be cancelled, or the venue changed, at short notice if weather conditions are unsuitable. If this occurs, alternative activities will be programmed.

• Please pack a jacket, hat, sunscreen (if sensitive to brands/types) and water bottle and ensure that all items are clearly named. To be “sun safe” children should wear tops that cover their shoulders.

• Sensible shoes should be worn (no thongs).

• Electronic devices should not be brought to vacation care and the service will not be responsible for lost or damaged items.

Registration Form which is available on the School website.
