Personal Information Forms

Information about early potential placements will be provided as they become known. / March / April / We will circulate details on early offers or Agency specific processes as they become available. Expressions of interest from students available for early placements / to start in May

If, for some reason you are no longer able to take up your allocated placement it is important to advise Linette immediately ( by 24 May at latest).
Field Educator to be advised of student referred / Wednesday, 26 May
From 27 May / Contact the agency to arrange an interview.
Take Field Educator kit and Placement Agreement Form to interview. Even if the decision is that the placement is not going ahead please leave the field educator kit at the agency.
June (onwards – as negotiated) / Placement begins
Placement Agreement Form / Once your Placement Agreement Form is completed and signed by you and the field educator/s, submit the form to Social Work Field Education Administration Officer (8.7.1) at least one week prior to beginning placement. If the form is faxed (9925 1855) or emailed the original Placement Agreement Form must also be submitted.
Placement Confirmation letter sent to student , field educator at agency and liaison staff member / . / Placement confirmation letter sent to agency which will advise student and field educator of liaison person
Draft Learning Plan / Student to send to liaison person at least three days prior to the first liaison visit
Semester 2
Integrative seminars / Dates to be advised / Attendance at the three integrative seminars is required
Final Report / 2 weeks after end of placement OR
23 November for students attending graduation. / Students who apply to participate in graduation ceremony must submit final report to Social Work Field Education Administration office (8.7.1) by 23 November.
Graduation Ceremony / 15th December 2010. / For those wishing to attend.
