HEALTH PLANNER
LINN COUNTY PUBLIC HEALTH DEPARTMENT

Promotes individual and population health by leading community health planning, policy, accreditation and performance management initiatives. Responsible for developing and implementing community health needs assessment and planning cycles in collaboration with Department staff, staff in other Linn County Departments, community stakeholders, elected officials, and community partners. Facilitates Department accreditation, quality improvement and performance management activities using best public health practices. Develops and/or implements new health programs; monitors, analyzes, and evaluates program effectiveness; and recommends program modifications/improvements. Reviews and analyzes new and proposed rules, regulations, and policies; assists the Epidemiologist and other staff in designing and implementing data collection systems; and researches, compiles, and analyzes data concerning health programs and services. Works under general supervision and/or as part of various workgroups and teams. Develops and implements timelines and master planning schedules for community health assessment and planning initiatives that flow from the community health action plan and/or the Department’s organizational strategic plan in order to assure the Department’s certification as a local health department in Iowa in a manner consistent with national public health accreditation standards. Facilitates and implements Departmental assessment and planning processes utilizing methodologies, including but not limited to Assessment Protocol for Excellence in Public Health (APEXPH) and Mobilizing for Action through Planning and Partnerships (MAPP) in order to develop community health action plans as required for certification. Researches and identifies potential plans/policies for opportunities for public health impact on issues relating to land use, the built environment and transportation in Linn County and the region. Participates in interdepartmental planning efforts in the development of county-wide policies and works with other County or City departments to plan strategies such as the land use and transportation plan that impact the built environment, transportation and public health of the County and the region. Coordinates training programs to build planning, policy development and environmental health outcomes awareness within the Department and throughout Linn County. Develops and implements a community involvement process that assures the active involvement of community members and stakeholders in the community health needs assessment and planning process. Participates in the Department’s design and application of the community health improvement strategies with community partners, members and stakeholders. Provides guidance and technical assistance to Department staff and community partners in community health needs assessment and planning. Contributes community health planning expertise to the Department’s community needs assessment process, health improvement goal setting, identification of public health policy and action options,
program development, and creation of monitoring and evaluation plans. Delivers targeted, culturally-appropriate information to help individuals and groups understand health promotion and disease prevention information, policies, regulations and local codes. Utilizes appropriate methods for interacting effectively and professionally with persons of all ages and from diverse cultural, socioeconomic, educational, racial and ethnic, sexual orientations, lifestyles and physical abilities. Performs public health emergency response duties as assigned and consistent with training provided, in response to threats to the public’s health consistent with job classification. Participates in Department and community emergency response training and drills in support of public health emergency and disaster preparedness. Participates in professional development activities by pursuing education, participating in professional committees and work groups and contributing to a work environment where continual improvements in practice are pursued. QUALIFICATIONS: Bachelor’s Degree in public health, health promotion or health education with a minimum of three (3) years of experience; OR a Master’s Degree in public health or urban/regional planning with one (1) year of experience. Must be able to obtain a Certified Health Education Specialist (CHES) credential within six (6) months of hire. Knowledge of key sources of available funding for public health initiatives. Demonstrated planning, teamwork and organizational abilities. Ability to build consensus and meet deadlines. Excellent interpersonal, communication and networking skills. Excellent writing skills; detail oriented. Computer skills, including MS Office Word, Excel, PowerPoint, Outlook and the ability to submit grant applications online. Ability to effectively synthesize large amounts of data and information; ability to develop SMART objectives. Demonstrated record of community based advocacy. Proven ability to work independently. Valid State of Iowa Driver's License required. Must pass a County physical examination which includes a drug test after offer of employment. Regular work attendance required.

SHIFT: 8:00 a.m. – 5:00 p.m. Monday – Friday 40 hours/week

SALARY: $21.27/hour
